摘 要:目的以CT、MRI为例,评价我国大型医用设备配置管理政策是否有效控制设备配置数量的无序增长。方法从我国东、中、西部抽取6个省(自治区、直辖市),从省级和地级的卫生行政部门和医疗机构层面收集设备配置信息,采用相关指标评价政策效果。结果从2003—2013年,样本省份的每百万人口的CT、MRI配置数量有所增长,配置量水平远低于同期OECD国家中位数水平;3样本省份CT、MRI设备的绝对数量以及每百万人口的配置数量,在政策实施后的年均增长率均低于政策实施前;与医院住院人次比较,政策实施后设备百万人口拥有量的增速在不同地区和不同设备间或高或低;部分省份医疗机构CT、MRI排队等候时间有偏长的现象。结论大型医用设备配置政策对于CT、MRI设备配置数量的无序增长起到了一定程度的抑制作用,但该项政策也面临一些挑战,需要进行相应的动态调整以适应我国基本国情的发展。Objective To evaluate the effect of China's Certificate of Need(CON) Policy on high technology medical equipment,using CT and MRI as an example. Methods Six provinces located in the east,middle and west regions in China were selected as the study sample. Questionnaires were used to collect the related information of high-tech medical equipment from local departments of health and hospitals. Results Although CT and MRI units per million inhabitants increased considerably in the study provinces from 2003—2013,they were still lower than the median level of OECD's countries in 2013. Both the average annual growth rates of CT and MRI decreased after the policy implementation. Compared with the growth rate of inpatient admissions,the growth rates of CT and MRI decreased after the policy. There is a significant difference in waiting time for CT or MRI across the provinces and hospitals. Conclusion The CON policy played an active role in controlling the growth rates of CT and MRI. However,it should be modified to adapt to the new development in China.
