组织运动三尖瓣环位移自动追踪技术对尿毒症患者右室收缩功能的评价

摘　　要：目的应用组织运动三尖瓣环位移法(TAD)测定三尖瓣环向心尖移动的距离,评价该技术对尿毒症患者右室收缩功能的应用价值。方法应用组织运动三尖瓣环位移法对45例尿毒症患者,其中左室肥厚(LVH)组25例,非左室肥厚(NLVH)组20例及30例正常对照组心尖四腔切面的三尖瓣环位移指标进行测定。结果尿毒症患者LVH组三尖瓣环位移、收缩期三尖瓣环连线中点的最大位移(TADmidpt),及其占右室长轴的百分比(TADmidpt%)较正常对照组显著性减低,NLVH组的三尖瓣环位移及其相关指标较正常对照组无显著性减低(*P<0.05,**P<0.01)。结论TAD提供了一种简捷、精准的评价急性尿毒症患者右室整体收缩功能的方法。Objective To evaluate the application value of tricuspid annular displacement （TAD） in assessment of the right ventricular systolic function in patients with uremia through determination of the tricuspid annulus to the apex displacement. Methods 45 patients with uremic including 25 patients with left ventricular hypertrophy （LVH）, 20 patients without left ventricular hypertrophy （NI.VH）, and 30 normal control subjects were measured with three groups of tricuspid annular displacement index in the apical four-chamber views by using （TAD）. Results Tricuspid annular displacement ,the maximum displacement of the midpoint connecting tricuspid annular （TADmidpt）, and the ratio of the maximum displacement of the midpoint accounted for length diameter of right ventricle （TMADmidpt%） in LVH group significantly reduced than the control group（P〈0.01and 0.05 respectively）. There was no significant decrease of TAD and its related index in NLVH group than those in the control group. Conclusions TAD is a kind of simple and accurate methods to evaluate global right ventricular systolic function in patients with uremia.
