儿童睾丸下降不全合并睾丸扭转的超声诊断

摘　　要：目的探讨超声在儿童睾丸下降不全合并睾丸扭转的诊断及其临床意义。方法回顾性分析我院3 424例睾丸下降不全中,8例经手术证实的睾丸下降不全合并睾丸扭转的手术病理结果及其声像图表现。结果在就诊的3 424例睾丸下降不全患儿中,经手术确诊合并睾丸扭转者8例;睾丸下降不全合并扭转的发生率约为0.23%。年龄2个月~14岁,其中〈1岁4例。5例的声像图表现为患侧不均匀类椭圆形低回声团块,彩色多普勒实质内无血流信号,术中睾丸均坏死行睾丸切除;1例正常睾丸声像图,但睾丸内及周围血流信号丰富,术中保留睾丸,行睾丸固定术。1例术前超声未显示睾丸声像图,仅提示腹股沟疝,手术显示睾丸坏死给予切除。1例超声提示睾丸下降不全伴畸胎瘤,行睾丸肿瘤剔除术并保留睾丸。结论儿童睾丸下降不全合并扭转不多见,超声诊断有助于发现危急值病例,为这类患儿的急诊手术治疗提供可靠的诊断依据。Objective To investigate the diagnostic value of ultrasound and its clinical significance in torsion of the undescended testis in children. Methods In this retrospective study we reviewed and analyzed 8 cases of torsion of the undescended testis confirmed by surgery from 3424 cases of undescended testis in children with pathologic findings and sonography at our hospital from. Results A total of 3424 patients presented with undescended testis, 8 of whom were operated confirmed with testicular torsion. The group of torsion of the undescended testis occur in approximately 0.23%. Patient age ranged from 2 month to 14 years （4 cases%l year）. 5 cases of sonogram showed heterogeneity oval hypoechoic mass, with no detectable Doppler flow in the testicle, then the orchideetomy were performed due to testicular gangrene. 1 case showed normal sized testicle with homogeneous echogenicity, with increased Doppler flow in the testicle and peripheral. The testicle was preserved and orchidopexy was performed at surgery. 1 case ultrasound diagnosed incarcerated hernia without evidence of normal testicle, but at surgery testicular gangrene was found and orchidectomy was performed. In the last case ultrasound diagnosed cryptorchid testicular teratoma, then the tumour enucleation was performed and testicle was preserved. Conclusions Torsion of the undescended testis is a relatively rare phenomenon that ultrasound is essential in making this diagnosis, and provide reliable diagnostic basis for emergency operation.
