摘 要:目的:探讨孤儿核受体NR2E1与新诊断2型糖尿病的关系。方法:研究对象分为糖尿病组(29名新诊断2型糖尿病患者)和对照组(48名健康志愿者)。测定研究对象身高、体重,抽取空腹静脉血测定血浆游离脂肪酸(FFAs)、甘油三酯(TG)、高密度脂蛋白(HDL-c)、空腹血浆胰岛素(FIN)、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、超敏C反应蛋白(hs-CRP)等临床指标,采用稳态模型评估胰岛素抵抗指数(HOMA-IR),分离外周血单个核细胞(PBMCs)实时荧光定量RT-PCR检测NR2E1 mRNA水平,Western blotting检测NR2E1蛋白水平,ELISA检测肿瘤坏死因子α(TNF-α)、白介素6(IL-6)表达水平;健康志愿者外周血PBMCs分别给予0、16.7mmol/L葡萄糖、250μmol/L棕榈酸刺激后培养24h,Western blotting检测NR2E1蛋白水平,ELISA检测上清中TNF-α、IL-6表达水平。分析糖尿病患者NR2E1表达水平和其他变量的相关性。结果:糖尿病组HDL-c较对照组明显降低(P<0.01),FIN、FBG、HbA1c、HOMA-IR、TG、FFAs、hs-CRP、TNF-α、IL-6表达较对照组明显升高(均P<0.01);糖尿病组NR2E1表达较对照组明显增加(P<0.01),且糖尿病组NR2E1mRNA与HbA1c(r=0.772,P<0.01),HOMA-IR(r=0.811,P<0.01)以及FFAs(r=0.752,P<0.01),hs-CRP(r=0.492,P<0.05)、TNF-α(r=0.625,P<0.01),IL-6(r=0.630,P<0.01)呈明显正相关。体外培养PBMCs经16.7mmol/L葡萄糖、250μmol/L棕榈酸刺激后NR2E1、TNF-α、IL-6表达较空白对照组明显升高(P<0.01)。结论:在新诊断2型糖尿病中,NR2E1的升高可能与糖脂毒性及2型糖尿病患者体内炎症状态有关。Objective:To evaluate the expression of NR2E1(nuclear receptor subfamily 2,group E,member 1)and its correlation with type 2diabetes.Methods:Plasma and peripheral blood mono-nuclear cells(PBMCs)were collected from 29 diabetic and 48 healthy individuals.The levels of FFAs,TG,HDL-c,FIN,and FBG,HbA1 c,and hs-CRP were measured.The insulin resistance index was calculated by using the homeostasis model assessment(HOMA).Levels of NR2E1 in PBMCs were analyzed by using real-time RT-PCR and Western blots.TNF-αand IL-6in plasma were measured by enzyme-linked immunosorbent assay(ELISA).PBMCs isolated from healthy volunteers were cultured with 0,16.7mmol/L glucose or 250μmol/L palmitate for 24 h,then levels of NR2E1 were tested by RT-PCR and Western blots,the concentration of TNF-αand IL-6in the supermatant were measured by ELISA.The relationship between NR2E1 and other variables was performed by using SPSS 19.0software.Results:FIN,FBG,HbA1 c,HOMA,and hs-CRP,TNF-α,IL-6were significantly different between diabetic patients and the control group.Levels of NR2E1 were higher in the diabetic group compared with the control group.NR2E1 expression was positively correlated to HbA1 cand HOMA,FFAs,hs-CRP,TNF-αand IL-6.The level of NR2E1 expression and the secretion of inflammatory cytokines increased under treatment with16.7 mmol/L glucose and 250μmol/L palmitatein cultureed PBMCs.Conclusion:Increased NR2E1 level may be closely associated with inflammation and disorder of lipid and glucose metabolism in diabetic patients.
