正常新生儿脑磁敏感加权成像相位位移值的分布规律评估

摘  要:目的:基于3.0T磁共振磁敏感加权成像(SWI)分析正常新生儿大脑相位位移解剖学规律及其与周龄间相关性,评价SWI在新生儿脑发育评估中的应用价值。方法:根据纳入标准收集80例3.0T MRI脑部SWI扫描正常新生儿,测量双侧深部灰质区、中央皮质区、额叶皮质区及顶叶皮质区相位位移值(PS);计算深部灰质区与大脑皮质区相位位移值平均值的比得到相位位移比率。SPSS 19.0版软件方差分析组间PS值差异;Spearsman相关性分析正常新生儿相位位移比率(PSR)与MRI检查时周龄相关性。结果:1正常新生儿脑深部灰质区PS均值为0.052±0.010;额叶大脑皮质区PS均值为0.059±0.010、中央大脑皮质区PS均值为0.045±0.009、顶叶大脑皮质区PS均值为0.039±0.009;额叶、中央区及顶叶大脑皮质相位位移值均值为0.048±0.009;深部灰质区PS高于大脑皮质平均PS(P<0.01);在大脑皮质中额叶区PS显著高于中央区和枕叶区(P<0.01)。2正常新生儿深部灰质区与大脑皮质区的PSR范围在0.139-2.746。PSR与MRI检查时周龄成负相关(r=-0.35,P=0.04)。结论:正常新生儿大脑PS存在解剖学差异,深部灰质区PS高于皮质区,皮质区PS从高到低为额叶皮质区、中央区和顶叶区;且深部灰质区和大脑皮质区相位位移比率随着妊娠并出生后周龄增长而不同程度降低;SWI的PS测量方法可无创性定量评估新生儿脑发育。Objective:To analyze phase shifts in anatomic diversities and age-related changes of the normal neonatal cerebrum based on 3.0TSWI,and to evaluate the application value of SWI in the neonatal brain development.Methods:Eighty normal neonates were collected with SWI scans on3.0T MR according to inclusive criterion.Phase shifts(PS)were measured in deep gray matter(DGM)and in the cortex(divided into bilateral frontal lobes,rolandic and occipital areas);Phase shift Ratios(PSR)were calculated.Intergroup PS value difference was analyzed by F variance on SPSS19.0version software,P〈0.05 was considered statistically significant;Correlations between PSR and gestational week-age at the time of MRI examination were assessed by Spearman correlation coefficients.Results:1The PS mean value of normal neonatals in the deep gray mat-ter was 0.052±0.010;The PS mean value was 0.059±0.010 in the frontal cortex,0.045±0.009 in the rolandic cortex and 0.039±0.009 in the occipital areas.PS mean value in the veins of the deep gray matter was significantly higher than that of the total cortical veins(P〈0.01).Among the cortical veins,the venous PS in the frontal area was the highest,following the rolandic and lowest occipital areas(P〈0.01).2The PSRs(ratio of the PS of the deep gray matter to that of the cortex)were 0.139-2.746.There was a negative correlation between the phase ratio and the gestational age at MRI(r=-0.35,P=0.04).Conclusion:In the normal neonates,an anatomical variation of PS is identified;PS of the deep gray matter is significantly higher than those of the other cortical veins.Among the cortex,the venous PS in the frontal area is higher than those of the rolandic and occipital areas.With gestational week-age increasing,PSRs of the deep gray matter and cortex gradually decrease.The PS measurement at SWI can effectively evaluate the brain development of neonates.
