财政补助对县级公立医院经济运行的影响:基于试点县的实证分析

摘　　要：目的:探索财政补助对试点县级公立医院2010—2014年经济运行状况的影响。方法:在东中西部各抽取两个省,共334个试点县的县级公立医院,用描述性分析、相关分析比较不同补助水平下医院的经济运行情况。结果:2010—2014年试点县级公立医院总体经济运行基本平稳,低补助组医院部分出现亏损;补助水平越高的医院,收支结构和负债情况相对更优,次均住院费用相对更低,医务人员收入也相对更高。结论:财政补助对医院的经济运行有显著影响,补助水平低的医院绩效较差,不当的财政补助方式可能加剧了医院的扩张行为并引发债务和职工积极性下降等问题,应建立财政补助的长效保障机制,改进财政补助资金的绩效管理,建立追踪问责机制。Objective： To explore the effect of financial subsidy on economic operation situation in the country-level public hospitals from 2010 to 2014. Methods： County-level hospitals in 344 pilot counties were selected from the eastern, central and western re- gions, 2 provinces for each region. Descriptive analysis and relative analysis were employed to compare the economic operation status of hospitals under different subsidy levels. Results： From 2010 to 2014, the economic operation of public hospitals at the county level was stable, with low subsidy group hospital losses; the higher the level of subsidy hospitals were, the better the balance of payments struc：ture and liabilities were, while the medical staff income was relatively higher. Conclusion： The financial subsidy of hospitals had remarkable effect on the economic： operation of hospital performance. Different fiscal subsidy method could enhance the expan- sion of hospitals anti cause problems of debts amt employees＇ enthusiasm. It should establish the long-acting security mechanism of fiscal subsidy, improve the performance management of fiscal subsidy fund and establish tracing accountability mechanism.
