从患者家属视角探讨江西省美沙酮维持治疗的疗效

摘　　要：目的从患者家属视角探讨江西省美沙酮维持治疗（MMT）效果,为MMT政策制定提供依据。方法在江西省13家MMT门诊,采用序贯法,随机抽取60%患者,再采用单纯随机抽样法,抽取30%患者家属进行相关调查,采用χ-2检验与秩和检验比较治疗前后疗效。结果共调查了457例MMT患者家属,女性占61.3%;29.5%为商业及服务业人员。MMT效果：（1）家庭功能：治疗后患者说谎、因吸毒受责备的频率均有明显降低（P〈0.05）,而帮做家务的频率明显提高（P〈0.05）,与家人的关系有明显改善（P〈0.05）。（2）社会功能：治疗后有工作的比例明显升高（P〈0.05）,与邻里的关系明显改善（P〈0.05）。（3）违法犯罪：偷、抢、骗等行为的发生率显著降低（P〈0.05）。（4）健康情况：治疗期间HIV新发感染率明显低于治疗前（P〈0.05）;总体健康状况明显改善（P〈0.05）。结论从MMT患者家属的视角看,江西省MMT取得了一定疗效,MMT工作有必要保持可持续发展。Objective To master the efficacy of methadone maintenance treatment（MMT） from the perspectives of patient family members in Jiangxi Province, and provide evidence to formulate MMT policies. Methods 30 percent family members of patients were chosen randomly by sequential method in the proportion of 60 percent selected in 13 MMT clinics of Jiangxi Province.Chi-square test and Wilcoxon rank sum test were used to compare the efficacy before and after the treatment. Results 457 family members of MMT patients were selected, and females accounted for 61.3%, 29.5% family members engaged in business and service industry. The main efficacy of MMT：（1） Family function： The frequency of lying to family, being blamed by family for taking drugs decreased significantly（P〈0.05）.While the frequency of helping family to do housework increased significantly（P〈0.05）. The relationship with family members improved significantly（P〈0.05）.（2） Social function： Employ rate increased significantly（P〈0.05）.And the relationship with neighborhood improved significantly（P〈0.05）.（3） Criminal activities： The frequency of stealing, robbing,swindle decreased significantly（P〈0.05）.（4） Health status： The infection rate of HIV during treatment was significantly lower than pre-treatment（P〈0.05）. The health status improved significantly after treatment（P〈0.05）. Conclusion MMT had chained good efficacy from the perspectives of family members in Jiangxi Province. The MMT should make long-time and sustainable development.
