摘　　要："和肝脾"是治疗肝脾不和型腹泻型肠易激综合征的主要方法。在《天癸病论与临床》"天癸至神理论"指导下重新构建辨治体系。指出天癸至神失调导致肝脾不和是本病的主要病机,其中至神失调是核心,肝脾不和是主因;当天癸至神失调失去对肝或脾的调节时,可出现脾虚肝乘、脾虚肝郁、肝气乘脾等三种不同情况;针对上述病因病机特点,提出"调至神、和肝脾"的创新治法。应用天癸至神理论指导治疗腹泻型肠易激综合征,对于认识本病病机、创新治疗方法、选择专病专药、提高临床疗效均具有重要的实用价值。The therapeutic method of harmonizing liver- spleen is one of the traditional Chinese medicine characteristic therapies for irritable bowel syndrome with diarrhea（ IBS- D） due to the disharmony of liver- spleen. This paper established the theoretical system of treatment based on syndrome differentiation of IBS- D under the guidance of the theory of supreme spirit of Tiangui illuminated by Treatise on Tiangui Disease and the Clinical Research. Main points are as follows： the disorder of supreme spirit of Tiangui causing disharmony of liver- spleen was the main pathogenesis of IBS-D,including the disorder of supreme spirit as the core pathogenesis and the disharmony of liver- spleen the main cause.When supreme spirit disorders lost the regulation of liver or spleen,it can cause spleen deficiency and liver- qi invading,liver depression and spleen deficiency,liver- qi invading the spleen and so on. In view of the above etiology and pathogenesis characteristics of IBS- D,we put forward the innovative therapeutic method of regulating supreme spirit and harmonizing liver- spleen. It is important practical value to guide the treatment of IBS- D by the theory of supreme spirit of Tiangui,including cognition of disease pathogenesis,innovation of therapeutic method,selection of special medicine and improvement on the clinical curative effect.
