消瘀泄浊饮对慢性肾衰竭营养不良大鼠瘦素以及残肾组织的影响

摘　　要：目的:观察消瘀泄浊饮对慢性肾衰竭(CRF)营养不良大鼠血清瘦素及残肾组织的影响。方法:46只SD雄性SPF级大鼠,随机选取正常组10只,其余大鼠采用5/6肾切除同时予低蛋白饮食制作CRF营养不良大鼠模型,观察其营养不良发生时间,随机分为消瘀泄浊饮组12只、复方ɑ-酮酸组(开同对照组)12只、模型组12只。治疗4周,分别检测肌酐、尿素氮、24 h尿蛋白、白蛋白、总胆固醇、甘油三酯水平,动态观察体质量变化,应用放射免疫法检测血清瘦素(Leptin)表达水平;应用光学显微镜(HE染色)观察肾组织病理学改变。结果:CRF大鼠在术后10周末出现营养不良,消瘀泄浊饮治疗组肾功能、24 h尿蛋白及营养状况明显改善(P<0.05或P<0.01),消瘀泄浊饮治疗组大鼠肾组织病理改变明显减轻,血清瘦素水平明显低于对照组(P<0.01)。结论:消瘀泄浊饮在保护CRF大鼠肾功能的同时,并可能通过下调血清瘦素水平,促进大鼠摄食,改善CRF大鼠营养不良。Objective： To investigate the effect of Xiaoyu Xiezhuo Decoction on leptin and remnant renal tissue in rats with chronic renal failure and malnutrition. Methods： 10 rats were randomly selected form the forty- six male SD rats as the normal group and the rest rats were eastablished into CRF- MN by 5 /6 nephrectomy and low protein diet. The happening time of MN in them was recorded. Rats successfully modeled were randomly divided into 3 groups： 12 rats in model group,12 rats in control group treated with composite ɑ- keto acid and 12 rats in Xiaoyu Xiezhuo Decoction treated group. After being treated for 4 weeks,serum creatinine（ Scr）,blood urea nitrogen（ BUN）,24 h- hour urine protein（ 24h- Upro）,triglyceride（ TC）,total cholesterol and albumin were detected and body weight in rats were observed dynamically. The blood leptin levels in rats were determined by radioimmunoassay and renal histopathological changes were examined by light microscopy（ HE staining）. Results： CRF rats revealed MN at the end of 10 th week after modeling. Compared with the model group,the condition of MN,renal function and 24 h- Upro of Xiaoyu Xiezhuo Decoction group were significantly inproved（ P〈0. 05 or P 〈0. 01）. Histopathological changes of Xiaoyu Xiezhuo Decoction group were significantly improved after treatment and the blood leptin level of Xiaoyu Xiezhuo Decoction group was significantly lower than that of the control group. Conclusion： Xiaoyu Xiezhuo Decoction can protect the renal function of CRF rats. At the same time,it can improve malnutrition condition,which is possibly by way of decreasing leptin level.
