右美托咪定与布托啡诺对前列腺电切患者术中及术后寒战疗效的比较

摘　　要：目的 :比较右美托咪定与布托啡诺对前列腺电切患者术中及术后寒战的治疗效果。方法 :收集择期在腰硬联合麻醉下行经尿道前列腺电切术术中及术后出现寒战的患者资料,60例患者根据使用药物不同分为右美托咪定组(A组)和布托啡诺组(B组),每组患者各30例。2组患者出现寒战后3min内A组给予右美托咪定0.5ug/kg持续泵注,B组静脉给予布托啡诺0.01mg/kg,观察2组患者用药前及用药后3min、5min、10min、20min、30min时HR、MAP、Sp O2变化,统计2组患者寒战等级、用药5min后治疗效果以及恶心、呕吐等不良反应发生情况。结果 :2组患者寒战分级无统计学差异,用药后2组患者寒战缓解或消失以及用药后的不良反应情况差异无统计学意义,但用药后2组患者HR、MAP相比,B组患者比A组更高,差异有统计学意义,2组患者SpO_2比较无统计学差异。结论:右美托咪定与布托啡诺对于前列腺电切患者术中及术后出现的寒战反应疗效相当,但右美托咪定更能保证血流动力学稳定。Objective： To compare the efficacy of dexmedetomidine and butorphanol for the prevention of shivering during resection of prostate. Method： for selective in the lumbar epidural anesthesia after transurethral resection of the prostate surgery and postoperative appear shivering patients,60 cases according to drug use different randomly divided into dexmedetomidine group （group A） and butorphanol group （B group）, with each group 30 cases. Shivering 3 minutes, A group given dexmedetomidine 0.5ug/kg continuous infusion,B group intravenous given butorphanol 0.01 mg/kg, observe two groups before treatment and after treatment in 3min and 5rain, 10min,20min,30min of the changes of HR,MAP, SpO2.Statistical chilis and efficacy after 5min, and adverse reactions. Results： Shiver classification in the two groups had no significant difference, chills alleviated or disappeared after treatment and the adverse reactions of differences without statistical significance, but HR, MAP of group B is higher than group A, the difference is statistically significant. SpO2 in the two groups has no significant difference. Conclusion： Two groups showed therapeutic equivalence, but dexmedetomidine set can guarantee the stability of hemodynamics.
