摘 要:目的探讨补肾活血方对激素性股骨头坏死大鼠病变早期血液粘滞性基因表达及病理生理改变的影响。方法成年健康SD大鼠30只,随机分成实验组,中药组以及对照组。实验组及中药组先予腹腔注射内毒素20g/kg,间隔24h,分2次注射,然后肌注甲强龙40mg/kg,连续注射3次,间隔24h。造模后中药组予补肾活血方灌胃,实验组及对照组均用等量0.9%氯化钠注射液灌胃,分别在造模后第3周处死大鼠,取大鼠左侧股骨头洗净、做矢状对半分开,一半行石蜡包埋、制备组织病理HE切片,另一半行基因芯片制作、分析及荧光定量PCR检测。结果组织病理学检测显示骨髓腔未见扩张,营养血管充盈情况良好,血管密度轻度增加。骨小梁间基质未见萎缩、退变,关节软骨厚度、细胞成熟度、生发层次序及生长板上中药组均好于实验组,并接近于正常对照组组织。基因芯片结果显示,中药组与实验组比较,ASPG,CRISPLD2和PLA2G2A的表达均明显增高(倍数变化〉2),差异有统计学意义;荧光定量PCR检测结果显示,中药组三种基因ASPG,CRISPLD2和PLA2G2A表达均显著高于实验组(P〈0.01),差异有统计学意义。结论激素性股骨头坏死早期相关血液粘滞性基因表达均出现明显下调.提示信号传导分子基因ASPG,CRISPLD2和PLA2G2A是早期的SNFH发生、发展过程中起作用的关键基因。补肾活血方能有效恢复股骨头血液粘滞性相关基因表达的改变,促进骨代谢动态平衡恢复,使早期股骨头病损得以及时修复。Objective To investigate the treatment of early Steroid-induced necrosis of femoral head ( SINFH ) by Bushen Huoxue decoction, and research the effect on the regulation changes of Gene expression of blood viscosity and correlation with pathomorphology. Methods 30 adult healthy SD rats were involved in this research and randomly divided into experimental group, Chinese medicine group and the control group. The experimental group and Chinese medicine group were intraperitoneally injected endotoxin 20g/kg per day for twice, and then intramuscular injected methyl prednisolone 40ml/kg per day for thrice. After building, Chinese medicine group was given Bushen Huoxue decoction 2ml per day by filling the stomach, while the experimental group and control group with the equiv^ilent 0.9% normal saline. All the rats were sacrificed in 3 weeks after building. The left femoral heads were stripped neatly and dissected immediately from the coronal plane, and then half of were preparing for the gene chip analysis and fluorescence quantitative PCR detection. The other half were prepared for paraffin embedding and sectioned and stained with H& E. Results Histopathological test showed the marrow cavity wasn' t expansion but with slight density increasing of blood vessel, blood supply was good, the situation no matter from articular cartilage thickness of femoral head, maturity of cartilage cells, germinal layer showed in Chinese medicine group were better than that of the experimental group, and close to the normal control group. According to the results of Gene chips, compared to the experimental group, the Chinese medicine group had a significant high expression of ASPG, CRISPLD2 and PLA2G2A ( fold change〉2 ) . The results of fluorescence quantitative PCR detection show that the Chinese medicine group of three genes ASPG, CRISPLD2 and PLA2G2Aexpression were significantly in the experimental group ( P〈0.01 ) .
