血清抵抗素与老年糖尿病心力衰竭的相关性研究

摘　　要：目的探讨抵抗素（Resistin）在老年2型糖尿病（T2DM）合并心力衰竭（HF）发病过程中可能机制及其与代谢指标的相关性。方法老年糖尿病患者70例分为单纯糖尿病组40例和合并心力衰竭组30例，另选择老年健康者35例为对照组。测定空腹血清抵抗素及相关代谢指标，用HOMA-IR评价胰岛素抵抗，采用NT-proBNP及EF值反映心脏功能，分析各指标间的相关性。结果老年T2DM合并心力衰竭组患者血清抵抗素水平均高于对照组及单纯糖尿病组，采用相关分析显示，老年T2DM合并心力衰竭组血清抵抗素浓度与HOMA-IR、NT-proBNP呈显著正相关（P〈0．05或0．01），与性别、年龄及体重指数等水平无相关性。结论老年T2DM合并心力衰竭患者血清抵抗素水平明显升高，提示过高水平的血清抵抗素可能通过某种机制和途径促进糖尿病患者的胰岛素抵抗水平，从而加速心力衰竭的形成。Objective Discussion on the pathogenesis of mechanism about the resistin in elderly patients with type 2 diabetes mellitus complicated with heart failure.At the same time, understanding its relationship with other metabolic indices. Method 70 patients with diabetes were divided into simple diabetes group 40 cases and complicated with heart failure group 30 cases, and 35 cases of elderly healthy control. Determination of serumresistin, fasting and metabolism index; evaluation of insulin resistance in HOMA-IR , using the NT-proBNP and EF values reflect the heart function, and then the analysising of correlation between different indexes. Result In elderly T2DM with heart failure patients, the serum resistin level average was higher than the normal control group and diabetes mellitus, the correlation analysis showed a significant positive correlation was found, elderly T2DM with heart failure group serum resistin concentration and HOMA-IR, NT-proBNP, and it had no correlation with the levels of sex, age and body mass index. Conclusion The serum resistin level is significantly elevated in patients with heart failure, the high level of serum resistin may promote the level of insulin resistance through certain mechanisms in patients with diabetes, so as to accelerate the formation of heart failure.
