摘  要:目的评价320排640层动态容积CT检测肾动脉解剖形态的优势及在介入治疗中的应用价值。方法回顾性分析200例中上腹部320排动态容积CT增强扫描动脉期资料,容积数据通过Vi TAL工作站进行后处理,结合容积再现、最大密度投影、多平面重建及曲面重建成像进行统计分析,检出肾动脉变异和疾病患者,并对肾动脉变异类型和病例进行分类分析。结果 200例患者中99例(49.5%)存在变异肾动脉,其中62例(31.0%)有副肾动脉,57例(28.5%)有肾动脉早发分支。共发现142支变异肾动脉,变异率为35.5%(142/400),左侧为38.5%(77/200),右侧为32.5%(65/200);副肾动脉80支(56.3%),肾动脉早发分支62支(43.7%);起源于腹主动脉78支,起源于肾动脉主干62支,起源于肠系膜上动脉1支,起源于右膈下动脉1支。200例患者中检出11例13支肾动脉狭窄,5例肾肿瘤。结论 320排动态容积CT增强检查扫描时间短,辐射剂量低,既能准确评估肾动脉变异类型,又能清晰显示各类变异细节,为临床肾脏病变介入治疗及其它外科治疗提供更多解剖学信息。Objective To investigate the application of 320-row dynamic volume CT scan in observing renal artery anatomy and in performing interventional therapy. Methods The arterial phase data of upper- middle abdomen 320- row dynamic volume enhanced CT scan of 200 patients were retrospectively analyzed. The volume data was post-processed by VITAL workstation. A statistical analysis of the images was conducted through volume rendering (VR), maximum intensity projection (MIP), multiplanar reconstruction (MPR) and curve-planar reconstruction (CPR). The renal artery variations and diseased patients were recorded, and the renal artery variation types and diseased patients were classified and analyzed. Results Of the 200 patients, renal artery variations were detected in 99 (49.5%), including accessory renal artery (n=62, 31%) and early branching of renal artery (n=57, 28.5% ). A total of 142 variant renal arteries were observed; the variation rate was 35.5% (142/400), with the variation rates of the left and right renal artery being 38.5% (77/200) and 32.5% (65/200) respectively. A total of 80 accessory renal arteries (56.3%) and 62 early branches of renal arteries (43.7%) were found. Of the 142 variant renal arteries, 78 originated from abdominal aorta, 62 from main renal artery, one from superior mesenteric artery and one from right inferior phrenic artery. Among the 200 patients, 13 stenosed renal arteries were found in 11 patients, and renal neoplasm was seen in 5 patients. Conclusion The upper-middle abdomen 320-row dynamic volume enhanced CT scan needs only short scanning time, besides, its radiation dose is low. This technique can not only accurately assess the renal artery variation types but also clearly display the variations in detail, providing more anatomical information useful for interventional and surgical therapy of renal lesions.
