摘　　要：目的探讨影响昆明地区新生儿脐血C肽的母亲相关因素,以早期进行干预改善新生儿结局。方法回顾性分析昆明医科大学第一附属医院产科2014年3-8月期间单胎活产分娩的840例产妇,分析影响新生儿脐血C肽的相关母亲并发症及人口统计学参数等一般资料。结果 1脐血C肽与母亲年龄、孕产次、民族、籍贯等一般情况及人体学参数无相关性;2妊娠期甲状腺疾病与脐血C肽存在正相关(r=0.81,P=0.023);3胎盘重量与脐血C肽存在正相关关系(r=0.025,P<0.001);4系统管理妊娠期糖尿病患者后,妊娠期糖尿病对新生儿脐血C肽值无影响。结论妊娠期甲状腺疾病可能与新生儿胰岛素抵抗相关,应加强对妊娠期甲状腺疾病的管理;系统管理妊娠期糖尿病患者,对改善新生儿胰岛素抵抗有显著的积极作用。Objective To explore maternal-related influencing factors of neonatal umbilical cord blood C peptide in Kunming region,conduct intervention and improve neonatal outcome early. Methods A total of 840 pregnant women（ single pregnancy） giving birth to their babies in the hospital from March to August in 2014 were analyzed retrospectively,the maternal complications and demographic parameters were analyzed. Results There was no correlation between umbilical cord blood C peptide and maternal age,gravidity,parity,nationality,native place,and human body parameters; gestational thyroid disease was positively correlated with umbilical cord blood C peptide（ r = 0. 81,P = 0. 023）; there was a positive correlation between placental weight and umbilical cord blood C peptide（ r = 0. 025,P〈0. 001）; after system management,gestational diabetes mellitus had no impact on neonatal umbilical cord blood C peptide. Conclusion Gestational thyroid disease may be associated with neonatal insulin resistance,gestational thyroid disease management should be strengthened; gestational diabetes mellitus cases should be managed systematically,which has active impact on improvement of neonatal insulin resistance.
