摘  要:目的比较平行于三尖瓣短轴位与左心室短轴位定位获得的右心室容积数据,评价前者的准确性和可重复性。方法分别采用平行于三尖瓣短轴位与左心室短轴位定位方法获得30名健康志愿者的心脏连续层面磁共振电影图像,测量右心室收缩末期容积、舒张末期容积、搏出量、射血分数,并进行比较,同时比较两种方法下左心室及右心室搏出量,并评价观察者间和观察者内差异。结果左心室短轴位与平行于三尖瓣短轴位定位方法所得右心室舒张末期容积[(117.4±23.8)ml,(125.6±25.2)ml]、收缩末期容积[(60.4±14.4)ml,(66.8±15.8)ml]、右心室搏出量[(57.1±12.7)ml,(58.8±12.8)ml]、右心室射血分数[(48.7±4.9)%,(46.9±4.8)%]均具有显著的统计学差异;除射血分数外,均以后者得到的容积更大。平行于三尖瓣短轴位定位方法所得右心室搏出量与左心室短轴位所得左心室搏出量无统计学差异,97%(29/30)数值位于一致性界限内。平行于三尖瓣短轴位定位方法所得右心室容积数据较左心室短轴位定位所得右心室容积数据具有更小的观察者间和观察者内差异,其偏差标准差和一致性界限更小。结论两种定位方法计算得到的右心室容积具有显著的统计学差异,平行于三尖瓣短轴位定位方法的可重复性更好。Objective To compare right ventricular volume measurements and their reproducibility between short axial paralleled to the tricuspid valve and left ventricle short axis orientation acquisition techniques. Methods Measurements of RV volumes from data sets acquired in short axial paralleled to the tricuspid valve and left ventricle short axis orientations were compared in 30 normal subjects. The observer variabilities were assessed and the left ventricle and right ventricular stroke volumes were compared. Results There was a significant difference in the right ventricular end-diastolic volumes, end-systolic volumes, right ventricular stroke volume and ejection fraction between left ventricle short axis and short axial paralleled to the tricuspid valve methods. The latter method resulted in larger volumes. The short axial paralleled tO the tri- cuspid valve method had lower intra- and interobserver variability than the left ventricle short axis method. There was no significant difference between right ventricular stroke volume acquired from short axial paralleled to the tricuspid valve method and left ventricular stroke volume acquired from left ventricle short axial. 97% (29/30)of the points were in the limits of agreement. The standard deviation of the difference and the limits of agreement were consistently lower for the short axial paralleled to the tricuspid valve method. Conclusion There is a significant systematic difference between volumes measured using the two different orientations. The short axial paralleled to the tricuspid valve orientation method result in better inter- and intraobserver reproducibility.
