CT引导下活检联合靶向治疗对肺部炎性病变的临床价值

摘　　要：目的探讨CT引导下活检联合靶向治疗在肺部炎性病变中的临床应用价值。方法搜集26例经活检确诊但临床治疗效果不佳的肺部炎性病变患者,再次行CT引导下活检联合靶向治疗,分析CT靶向治疗效果,并进一步提高活检的阳性率。结果 26例中,有21例检查结果与初诊结果一致,治疗后复查显示,16例治愈或好转,3例无明显变化,2例进展;5例诊断结果与初诊不相符,其中,合并肺腺癌2例,小细胞肺癌1例,肺真菌感染2例。并发症为少量气胸2例,少量咯血3例,没有发生致命性并发症。结论 CT引导下活检联合靶向治疗对临床保守治疗效果不佳的肺部炎性病变有着重要的临床意义。Objective Evaluate the clinic value to combine the percutaneous biopsy with target therapy under CT guid- ance in lung diseases. Methods The lung cases that were confirmed by percutaneous biopsy, and The efficacy of that was not good by clinical treatment, were collected. These cases perfomed biopsy and CT-guided target therapy, and evalua- ted the effect of the targeted therapy technique, further confirmed clinical effectiveness and enhanced diagnosis rate. Re- suits Out Of 26 cases, the results of 21 cases were the same to the initial diagnosis, and the treatment results showed that 16 lesions disappeared completely or was obviously diminished, 3 had no significant changes in size, and 2 had progressed. The results of 5 cases were not the same as the initial diagnosis. The results of 5 cases were lung adenocarcinoma, 2 were small-cell lung cancer, 1 was lung fungal infection. As for complications, 2（7.7% ：2/26） cases suffered pnemothroax, 3 （ 11.5% ：3/26） cases experienced mild hemoptysis. No fatal complications occurred. Conclusion combining the percu- taneous biopsy with target therapy in the diagnosis and treatment of lung disease has a more important clinical significance.
