医院内下呼吸道感染病原菌分布及耐药性分析

摘　　要：目的:探讨分析医院内下呼吸道感染病原菌的分布及耐药性.方法:采集患者晨痰为标本,接种至血琼脂平板,35℃培养24 h,采用DL-96细菌测定系统进行细菌鉴定,K-B琼脂扩散法进行药敏分析,进行结果判断.结果:1210份标本培养后共分离出348株菌株,病原菌检出阳性率28.8%,病原菌主要分布为肺炎克雷伯菌、铜绿假单胞菌、鲍氏不动杆菌、肺炎链球菌和金黄色葡萄球菌;肺炎克雷菌对左氧氟沙星的耐药率最高,铜绿假单胞菌对青霉素的耐药率最高,鲍氏不动杆菌对青霉素的耐药率最高,肺炎链球菌对红霉素的耐药率最高,金黄色葡萄球菌对青霉素G的耐药率最高.结论:下呼吸道感染病原菌主要以革兰氏阴性菌为主,不同菌株对抗菌药物的耐药性不同,临床治疗需根据病原学检查结果,合理选择抗感染药物.AIM： To investigate the distribution and drug resistance of pathogenic bacteria in lower respiratory tract infection in hospital. METHODS： The morning sputum from the patients were collected as samples,and inoculated to blood agar plate,and the bacteria were cultured for 24 hours at 35℃. The bacterial identification was carried out by DL-96 method. The K-B agar diffusion method was used for drug sensitivity analysis.RESULTS： Total separation cultured 348 strains after cultivation in 1210 specimens,and pathogenic bacteria detection positive rate was 28. 8%. Pathogenic bacteria mainly distributed for pathogen Klebsiella pneumoniae,Pseudomonas aeruginosa,Acinetobacter baumannii,Streptococcus pneumoniae and Staphylococcus aureu Klebsiella pneumoniae strains have highest resistance to levofloxacin;Pseudomonas aeruginosa strains have the highest resistance to penicillin; Acinetobacter baumannii strains have the highest resistance to penicillin; Streptococcus pneumoniae strains have the highest resistance to erythromycin,and Staphylococcus aureus strains have the highest resistance to penicillin G. CONCLUSION： The main pathogenic bacteria in lower respiratory tract infections is Gram-negative bacteria,different strains have different drug resistance to antimicrobial drug. Clinical treatment should be made according to the etiology examination results,and choose reasonable anti-infective drugs.
