2型糖尿病患者血清同型半胱氨酸含量与糖尿病肾病关系的探讨

摘　　要：目的探讨2型糖尿病(DM2)患者同型半胱氨酸含量与糖尿病肾病的关系。方法将80例DM2患者分为同型半胱氨酸含量正常组和升高组,另以40例健康体检者作为正常对照组,以酶法检测血清同型半胱氨酸、免疫比浊法检测血清Cys C、免疫透射比浊法检测尿Alb含量,并计算尿Alb/Cr值。结果 (1)DM2患者Hcy正常组与正常对照组Hcy和Cys C比较无显著性差异(P>0.05);(2)DM2患者Hcy升高组与DM2患者Hcy正常组及正常对照组Hcy和Cys C均有显著性差异(P<0.05);(3)三组Alb/Cr之间均有显著性差异(P<0.001);(4)DM2患者血清Hcy含量与Cys C、Alb/Cr呈显著正相关(P<0.01)。结论 DM2患者Hcy含量与糖尿病肾病(DN)有一定的相关性,在防治糖尿病肾病时应注意血清同型半胱氨酸含量的调控。Objective To explore the relationship between serum homocysteine （Hcy）concentration and diabetic nephropathy in patients type 2 diabetes. Methods The study included 80 cases of type 2 diabetes patients in which 40 patients with normal Hcy concentration and 40 patients with elevated Hcy concentration, and 40 healthy volunteers were selected as controls. The serum Hcy and CysC in patients and controls were detected by enzyme method and ITM respectively. The urine Alb was detected by Bromocresol green method, and alb/cr was calculated. Results There was no significant difference between type 2 diabetes patients with normal Hey concentration and control group the group on serum levels of Hey and CysC （ P 〉 0.05 ）. The serum Hcy and CysC levels in type 2 diabetes patients with elevated Hcy group were significantly higher than that of in type 2 diabetes patients with normal Hey group and control group （P 〈 0.05 ）. There was significant difference among the three groups on the ratio of alb/cr（P 〈0. 001 ）. The sermn Hey levels in type 2 diabetes patients was positively correlated with serum Cys C levels and Alb/cr ratio（P 〈0.01 ）. Conclusion The serum Hcy level in patients with type 2 diabetes was correlated with diabetic nephropathy, and it should be controlled in the prevention and treatment of diabetic nephropathy.
