清开灵对银屑病患者皮损面积、PASI评分及外周血Th1/Th2平衡的影响

摘　　要：目的探讨清开灵对银屑病患者皮损面积、严重性指数(PASI)评分及外周血Th1/Th2平衡的影响。方法将100例寻常型银屑病患者按随机数字表法分为对照组和观察组,每组50例。2组均给予内服中药汤剂土槐饮加减并外用湿毒软膏,观察组在上述治疗基础上给予清开灵注射液静滴,对照组不给予其他治疗。2组均以2周为1个疗程,连续治疗2个疗程。统计2组临床疗效,观察2组治疗前及治疗1个疗程、2个疗程后PASI评分及血清细胞因子肿瘤坏死因子α(TNF-α)、干扰素-γ(IFN-γ)、白细胞介素-4(IL-4)、IL-10水平变化情况。结果观察组治疗后总有效率明显高于对照组(P<0.05)。治疗后2组PASI评分均较治疗前明显降低(P均<0.05),且治疗2个疗程后明显低于治疗1个疗程后(P均<0.05);观察组治疗1个疗程、2个疗程后的PASI评分均明显低于同期对照组(P均<0.05)。治疗后2组血清TNF-α、IFN-γ水平均明显降低(P均<0.05),且观察组较对照组降低更为明显(P均<0.05);而2组血清IL-4、IL-10水平均变化不大,治疗前后及组间比较差异均无统计学意义(P均>0.05)。结论清开灵注射液治疗银屑病疗效显著,能有效改善患者的临床症状,有效纠正外周血Th1/Th2细胞因子失衡状态,值得在临床上广泛应用。Objective It is to investigate the effect of Qingkailing injection on the skin lesion area, PASI score and peripheral blood Th1/Th2 balance in patients with psoriasis vulgaris. Methods 100 patients with psoriasis vulgaris were randomly divided into control group and observation group, 50 cases in each group. All patients were given oral medicine decoction soil Huai drink （plus or minus） and topical ointment Shidu （hospital internal preparations） , and then the patients were given Qingkailing injection based on the above in observation group, and not given other treatment in the control group. Two weeks for a period of treatment in both groups, and treated for two periods. The clinical curative effect of two groups was compared. The score of PASI and the levels of serum TNF - α, IFN - γ, IL - 4, IL - 10 cytokine were detected in two groups before treatment and after 1 and 2 course of treatment. Results The total effective rate in observation group was significantly higher than that in control group （χ^2 = 9.01 ,P 〈 0.05 ）. The PASI score after treatment in two groups was significantly decreased than before treatment （all P 〈 0.05 ）, and those continue to decrease as the course goes on; the PASI score of observation group after treatment were significantly lower than those of control group （ all P 〈 0.05 ）. The levels of serum TNF -α, IFN -γ cytokine were significantly reduced in two groups after treatment （ all P 〈 0.05 ） , and the reduction of observation group was more significant （P 〈 0.05） ; the levels of IL- 4, IL- 10 cytokine levels was little changed, there was no significant difference between both groups or before and after treatment （ P 〉 0.05 ）. Conclusion The effect of Qingkailing injection in treating psoriasis is very significant, it not only can effectively improve the clinical symptoms, but also to effectively correct the peripheral blood Th1/Th2 cytokine imbalance, worthy of clinical widely used.
