牙周炎与高血压前期的相关性研究

摘　　要：目的:了解牙周炎患者高血压前期的发病情况,分析牙周炎的严重程度与高血压前期的关系,探讨炎性细胞因子在牙周炎伴高血压前期中的作用。方法:对310例牙周炎患者进行血压测量和牙周检查,调查牙周炎患者高血压前期的发病情况,分析轻、中、重度牙周炎与高血压前期的关系;随机选择单纯牙周炎43例,牙周炎伴高血压前期40例,伴高血压38例,测定受试者血清中的超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)和肿瘤坏死因子(TNF-α)水平,分析上述指标之间的差异。结果:牙周炎患者中高血压前期的发病率为42.26%,男性高于女性(P<0.01);中、重度牙周炎伴高血压前期的发病率分别为36.64%和42.75%,显著高于轻度牙周炎伴高血压前期的发病率(P<0.05或<0.01);牙周炎伴高血压前期组的血清hs-CRP、IL-6、TNF-α水平显著高于单纯牙周炎组(P<0.05或P<0.01);多元逐步回归分析显示,中、重度牙周炎与高血压前期呈独立相关(P<0.05),血清hs-CRP、IL-6、TNF-α水平与牙周炎伴高血压前期呈独立相关(P<0.05)。结论:牙周炎患者高血压前期的发病率较高;炎性细胞因子与牙周炎伴高血压前期密切相关。Objective：To investigate the correlation between the periodontitis and prehypertension by detecting the prehypertension rate,the periodontal status and serum levels of inflammatory cytokines with periodontitis patients.Methods：The 310 patients with periodontitis were detected to analyse the prehypertension incidence by checking blood pressure,and to explore the relationship between the prehypertension and periodontitis patients of the different status.There were 43 patients with pure periodontitis,40 patients with periodontitis and prehypertension,38 patients with periodontitis and hypertension.The serum levels of inflammatory cytokines including hypersensitivity C reactive protein（hs-CRP）,interleukin-6（IL-6）and tumor necrosis fact（TNF-α）were measured.Results：The incidence of prehypertension with periodontitis patient was 42.26% and more common among men than women（P〈0.01）,while the incidences of prehypertension with moderate and severe periodontitis were36.64% and 42.75%respectively and there was significant higher than that of patient with mild periodontitis（P〈0.05or〈0.01）.The serum levels of hs-CRP、IL-6and TNF-αof the periodontitis patients with prehypertension was significantly higher than those with pure periodontitis sbujects（P〈0.05 or P〈0.01）.Multiple stepwise regression analysis showed that moderate and severe periodontitis were correlated to prehypertension independently（P〈0.05）and hs-CRP,IL-6and TNF-αwere correlated to prehypertension with periodontitis independently（P〈0.05）.Conclusion：The incidence of the prehypertension with periodontitis patients is high and increases with the aggravation of the periodontitis.Inflammatory cytokines may be an important role in the prehypertension with periodontitis patients.
