B7-H3和Toll样受体4分子在结直肠癌组织中的表达及其临床意义

摘　　要：目的探讨B7-H3和Toll样受体4（TLR4）分子在结直肠癌组织中的表达及其临床意义。方法收集142例结直肠癌患者的肿瘤组织及临床资料，免疫组织化学方法检测B7-H3和TLR4分子在肿瘤组织中的表达，并对结果进行统计学分析。结果肠癌组织中B7-H3分子的表达与淋巴结转移和远处转移明显相关（P〈0．05），TLR4分子的表达与淋巴结转移明显相关（P〈0．05）。B7-H3和TLR4分子的表达有相关（P〈0．05）。B7-H3与TLR4共同高表达44例，B7-H3高表达而TLR4低表达者23例，B7-H3低表达而TLR4高表达36例，共同低表达39例。生存分析结果显示肿瘤组织中B7-H3或TLR4分子表达水平低的患者其生存率显著高于表达水平高的患者（P〈0．05）。结论B7-H3和TLR4的表达水平与结直肠癌临床特征相关，可能参与了结直肠癌的免疫病理过程。Objective To analyze the expression and clinical significance of B7 - H3 and Toll like receptor 4 （TLR4） in colorectal carcinoma tissue. Methods Chnical data and surgical frozen tumor tissues of 142 patients with colorectal carcinoma were recruited from January to December 2003 in the Oncology Department, Affiliated Hospital of Jiangnan University and the Third Affiliated Hospital of Soochow University （China） in the present study. The expression of B7 -H3 and TLR4 in colorectal carcinoma tissues was detected by immunohistochemistry. Relationship between the expression of B7 - H3 and TLR4 and clinical parameters of patients was analyzed statistically. Results B7 - H3 expression in colorectal carcinoma tissues was significantly associated with lymph node metastasis and distant metastasis （ all P 〈 0. 05 ）, and TLR4 expression was significantly associated with lymph nodes metastasis （ P 〈 0. 05 ）. Furthermore, B7 - H3 expression was correlated with TLR4 expression （Х^2 =4. 493 ,P 〈0. 05）. The patients of B7 - H3 high co - expressed with TLR4 were 44 cases, B7 - H3 high expression and low expression of TLR4 were 23 cases, B7 - H3 low expression and high expression of TLR4 were 36 cases, low co - expression of B7 - H3 and TLR4 was 39 cases. The log - rank survival analysis showed that the survival rate of patients with low expression of B7 - H3 or TLR4 was obviously higher than in those with high B7 - H3 or TLR4 expression. Conclusion The expression of B7 - I-I3 and TLR4 was associated with clinical parameters of colorectal carcinoma patients, and they maybe take part in immunopathologic mechanism of colorectal carcinoma.
