摘  要:目的观察氟比洛芬酯对神经病理性疼痛大鼠脊髓背角神经元凋亡的影响。方法成年健康雄性SD大鼠54只,体质量200~250g,采用随机数字表法,将其随机分为3组(n=18):假手术组(S组)、神经病理性痛组(NP组)和氟比洛芬酯组(F组)。S组仅分离坐骨神经但不结扎,NP组和F组采用坐骨神经慢性压迫损伤法建立神经病理性痛模型。F组于术毕开始腹腔注射氟比洛芬酯10mg/kg,1次/日,连续14d;S组和NP组以等容量生理盐水替代。于术前1d、术后3、7、14d时测定热痛阈(TWL),并于术后3、7和14d时测定痛阈后各取6只大鼠处死,取脊髓背角,采用免疫组织化学法测定半胱氨酰天冬氨酸特异性蛋白酶-3(Caspase-3)和B细胞淋巴瘤/白血病-2(bcl-2)的表达水平,采用原位缺口末端标记法(TUNEL)法检测脊髓背角凋亡神经元细胞,计算凋亡率。结果与S组比较,术后3、7、14dTWL在NP组[(5.24±0.52)、(3.54±0.47)、(3.84±0.55)s]和F组[(5.68±0.61)、(7.76±0.71)、(9.68±0.81)s]缩短,脊髓背角神经元细胞凋亡率在NP组[(9.24±1.22)%、(9.54±1.37)%、(9.84±1.45)%]和F组[(6.68±0.91)%、(6.26.4±1.08)%、(6.15±0.83)%]升高,Caspase-3表达术后3、7、14d在NP组(1.34±0.16、1.45±0.18、1.38±0.16)和F组(0.96±0.09、1.02±0.12、0.89±0.08)上调,bcl-2表达在hip组(0.36±0.04、0.45±0.03、0.32±0.05)和F组(0.68±0.06、0.75±0.07、0.89±0.08)下调(P〈0.05);与NP组比较,F组TWL[(5.68±0.61)、(7.76±0.71)、(9.68±0.81)s]延长,脊髓背角神经元细胞凋亡率[(6.68±0.91)%、(6.26±1.08)%、(6.15±0.83)%]降低,Caspase-3表达(0.96±0.09、1.02±0.12、0.89±0.08)下调,bcl-2表达(0.68±0.06、Objective To investigate the effects of flurbiprofen axetil on the neuronal apoptosis in the spinal dorsal horns in a rat model of neuropathic pain (NP). Methods Fifty - four male SD rats weighing 200 -250 g, were randomly divided into 3 groups (n = 18 each) : sham operation group (S group), NP group and flurbiprofen axetil group ( F group). The animals were anesthetized with intraperitoneal 10% chloral hydrate 350 mg/kg. In NP and F groups, 4 ligatures were placed on the right sciatic nerve at I mm intervals with 4 -0 silk thread according to the method described by Bennett et al. In S group, the right sciatic nerves were exposed, but not ligated. In F group, flurbiprofen axetil 10 mg/kg was injected intraperitoneally once a day starting from the end of operation until one day before the animals were sacrificed, while the rats received the equal volume of normal saline (5 ml/kg) instead of flurbiprofen axetil in S and NP groups. Animals were chosen at 1st day before operation and 3rd, 7th and 14th day after CCI (T0 - T3 ) to measure paw withdrawal latency to thermal nociceptive stimulus (TWL). The animals were killed on the postoperative day (POD)3, 7 and 14. The lumbar segment (IA -6) of the spinal cord was imme- diately removed. The expression of Caspase - 3 and B cell lymphoma/leukemia - 2 (bcl - 2) in the dorsal horns of spinal cord was measured by immunohistochemistry, and the neuronal apoptosis rate was determined by TdT - mediated dUTP nick end labeling (TUNEL). Results As compared with S group, TWL was shortened in NP group [ (5.24 ±0. 52), (3. 54 ±0. 47), and (3. 84 ±0. 55) s] and F group [ (5. 68 ± 0.61), (7.76±0.71), and (9.68±0.81) s] on the POD 3, 7, andl4 (P〈0.05), the neuronal apoptosis rate was significantly increased in NP group [ ( 9.24 ± 1.22 ) % , ( 9. 54 ± 1.37 ) % , and (9.84± 1.45)%] and F group [(6.68 ±0.91)%, (6.26 ±1.08)% and (6. 15 ±0.83)%], and Caspase-3 expression was signi
