摘 要:目的探讨能谱CT成像定量分析小肠活动性出血的价值。方法将离体猪小肠与微量注射器连接,分别设置5种注射流率(0.1、0.2、0.3、0.4、0.5ml/min)模拟活动性小肠出血。对不同出血流率的猪小肠进行能谱成像(GSI),分别延迟15、40s扫描模拟动脉期和门静脉期。利用能谱CT基物质碘基图,分别测量动脉期肠腔内总碘含量值(IC动脉期)及门静脉期总碘含量值(IC门静脉期),并利用公式计算出血流率[V测定=(IC门静脉期-IC动脉期)/(Δt×ρI)],初步估计小肠活动性出血量,分析真实流率与测定流率间的关系。结果真实注射流率分别为0.1、0.2、0.3、0.4、0.5ml/min的模型测定流率分别为0.071 2、0.256 6、0.358 7、0.468 5、0.559 4ml/min;测定流率与真实流率间的误差率分别为28.77%、28.30%、19.57%、17.13%、11.88%。经过回归分析测定流率与真实流率呈正相关(r=0.984,P=0.002)。结论利用能谱CT碘基图像测量碘含量计算小肠活动性出血流率,并以此评估小肠活动性出血量具有一定的可行性。Objective To explore the value of energy spectral CT in quantitative diagnosis of the small intestine active bleeding. Methods The bleeding model was established using the small intestines of pigs in vitro. Five blood flow rates (0.1, 0.2, 0.3, 0.4, 0.5 ml/min) were simulated. Gemstone spectral imaging (GSI) was performed in the bleeding model with different blood flow rates and a delay of 15 s and 40 s simulated the arterial phase and portal venous phase, respectively. The amount of iodine were measured in the arterial (ICarteriar) and portal venous phase (ICportal) with iodine-based image of energy spectral CT, then the rates of blood flow were calculated according to formula (V= [ICportal- ICarterial]/[A t)〈 ρI]) to preliminary estimate intestinal active blood loss. Relationship between the actual flow rate and the measured flow rate was analyzed. Results The actual injection flow rate were 0.1, 0.2, 0.3, 0. 4, 0.5 ml/min and the experimental flow rate were 0. 071 2, 0. 256 6, 0. 358 7, 0. 468 5, 0. 559 4 ml/min. Measurement error rate between flow rate and the actual flow rate were 28.77%, 28. 30%, 19.57%, 28.30%, 11.88%. There had positive correlation between the actual injection flow rate and the experimental flow rate (r=0. 984, P= 0. 002). Conclusion It is feasible using the iodine-based image of ener- gy spectral CT to measure iodine content in small intestine out of the blood flow rate and to evaluate intestinal active blood loss.
