摘  要:目的采用血流向量成像技术(VFM)探讨高血压(HP)和主动脉瓣关闭不全(AR)患者的左心室舒张末期平均能量损耗(AELEDLV)对左心房前后径及室间隔厚度的影响。方法将患者分为正常组、AR组、HP组及HP+AR组,每组15例。采用VFM分析各组心电图R波顶点四腔心的AELEDLV,测量舒张期末室间隔厚度和左心房前后径。结果 HP+AR组AELEDLV显著高于HP组、AR组及正常组(P均<0.01),HP组及AR组高于正常组(P<0.01);AELEDLV、左心房前后径及室间隔厚度均为HP+AR组>HP组>AR组>正常组,AELEDLV与左心房前后径呈正相关(r=0.98,P=0.025)。结论 AR及HP均可增加AELEDLV,HP合并AR可进一步增加AELEDLV,AELEDLV的增加可能是导致左心房增大及室间隔增厚的重要因素之一。Objective To investigate the influence of the average energy loss of end-diastole in left ventricle (AELEDLV) with hypertension (HP) and aortic valve regurgitation (AR) on the interventricular septum (IVS) thickness and the left atrium (LA) diameter with vector flow mapping (VFM). Methods All subjects were divided into control group (n=15), AR group (n=15), HP group (n= 15) and HP+AR group (n= 15). VFM was used to analyze the AELEDLV of the four-chamber view at R wave of the ECG. IVS thickness and LA diameter were measured at the end diastolic period. Results AELEDLV of the HP+AR group was the highest than that of the HP group, AR group and the control group (P〈0.01). AELEDLV of the HP group and the AR group were higher than that of the control group (P〉0.01). The mean values of AELEDLV, the LA diameter and the IVS thickness of the HP+ AR group were the highest and those of the control group were the smallest among all groups, those mean values of HP group were more than those of AR group, AELEDLV was correlated with the LA diameter (r = 0.97, P = 0. 025). Conclusion AR or HP could increase AELEDLV, HP and AR could further increase AELEDLV, and the increase of AELEDLV is one of the important factors that can cause the LA enlargement and the IVS thickness probably .
