超声心动图诊断完全型肺静脉异位引流及合并心脏畸形

摘　　要：目的探讨超声心动图诊断完全型肺静脉异位引流(TAPVD)及合并心脏畸形的应用价值。方法回顾性分析经我院超声心动图诊断为TAPVD的59例患儿的临床及超声心动图诊断资料,其中12例于本院接受CTA检查,19例于本院接受手术治疗,对余28例进行电话随访。结果 59例TAPVD患儿中,心上型24例(24/59,40.68%),心内型22例(22/59,37.29%),心下型8例(8/59,13.56%),混合型5例(5/59,8.47%)。59例患儿均合并房间隔缺损,其中10例共同肺静脉伴有不同程度的狭窄。除8例失访外,在接受CTA检查与手术治疗的51例患儿中,50例超声心动图检查结果得到证实,余1例误诊。超声心动图诊断符合率为98.04%(50/51),诊断引流位置准确率为100%(50/50)。结论超声心动图对诊断TAPVD分型及合并心脏畸形具有重要的应用价值。Objective To explore the application value of echocardiography in diagnosis of total anomalous pulmonary venous drainage （TAPVD） and associated cardiac malformations. Methods Clinical and echocardiography diagnostic datum of 59 cases with TAPVD confirmed by echocardiography were retrospectively reviewed. Twelve cases underwent CTA, 19 cases underwent surgery in our hospital, and 28 cases were followed up by telephone, Results Among 59 cases, there were 24 cases （24/59, 40. 68%） of supra-cardiac type, 22 cases （22/59, 37. 29%） of intra-cardiac type, 8 cases （8/59, 13.56%） of inffa-cardiac type and 5 cases （5/59, 8. 47%） of mixed type. All of the 59 cases had atrial septal defect, and 10 of them had stenosis of common pulmonary vein. Except for 8 cases lost to follow up, 51 cases underwent CTA or surgery, and echocardiography diagnostic results of 50 cases were confirmed, 1 case was misdiagnosis. The diagnostic accordance rate of echocardiography was 98.04% （50/51）, and the accuracy rate of the drainage position was 100% （50/50）. Conclusion Echocardiography has an important application value in diagnosis of TAPVD and associated cardiac malformations.
