冠状动脉钙化积分评价冠状动脉弹性的研究

摘　　要：目的 探讨冠状动脉钙化积分（CS）与冠状动脉弹性之间的相关性以及冠状动脉CS评价冠状动脉弹性的可行性。方法 选择冠心病患者30例，其主要病变位于左前降支。对照组为疑似冠心病（如胸闷、胸痛）但冠状动脉造影检查未见明显狭窄者30例行CT血管造影（CTA）检查，2组年龄均＜55岁。应用冠状动脉CTA测量和计算所有研究对象左前降支的弹性指标——弹性系数值（D value），并对其钙化灶进行峰值计分，应用计算机工作站直接进行计算得到CS，分析弹性系数值和CS之间的相关性。结果 病例组左前降支弹性小于对照组，差异具有统计学意义（P〈0.05）。病例组左前降支血管的CS平均为337.52±84.43，大于对照组的平均值154.56±62.54，差异有统计学意义（P〈0.01）。Pearson相关性分析显示病例组和对照组左前降支血管的CS与弹性系数值都在0.01水平上显著相关，相关系数分别为-0.884和-0.991，为负相关。结论 CS与冠状动脉血管弹性呈现良好的负相关关系，当CS增大时，该血管的弹性下降。CS评价冠状动脉弹性具有可行性，可为临床上冠状动脉弹性的评价提供参考。Objective To explore the relationship between the coronary calcium score and the coronary distiensibility,and to ex plore the feasibility of evaluating the coronary distensibility by coronary calcium score. Methods Computed tomographic angiogra- phy （CTA） was performed in 30 control adults who were suspected coronary heart disease with chest tightness and chest pain,how- ever,coronary angiography revealed negative. CTA was also performed in 30 patients with CAD. All subjects were younger than 55 years. The main lesion located in the left anterior descending branch. The distensibility value （D value） of left anterior descending ar- tery was determined and the coronary calcium score （CS} was measured of all the subiective with CTA,and then the relationship between the distiensibility value and the calcium score was analyzed. Results There was a significant difference in the D value between normal control subjects and patients （P〈0.05）. Compared to normal controls, the coronary calcium score of the LAD artery in the patient group showed significant difference（P〈0.01 ）. There was a significant correlation in the patient group along with the control group between the coronary calcium score and the coronary distensibility of LAD at the level 0.01, and the correlation coefficients were -0. 884 and 0. 991 ,respectively. Conclusion The coronary calcium score and the coronary distensibility show apparently inverse correlation. The coronary calcium score can be applied clinically as a reference in the evaluating of the coronary distensibility.
