摘 要:目的从3个方面探讨Sky骨扩张器与球囊椎体后凸成形术(PKP)对椎体复位的作用进行比较研究:(1)椎体生物力学性质的改善情况;(2)椎体高度的恢复情况;(3)术中骨水泥向椎体外渗漏情况。方法取20个胸腰段椎体(老年骨质疏松性椎体),经万能力学试验机轴向加载,制造椎体压缩骨折模型,随即行Sky骨扩张器与球囊PKP,测定椎体在骨折前、后及骨水泥注入后的高度并记录骨水泥注入前、后最大负荷,计算其刚度。结果Sky骨扩张器组与球囊组均能增加椎体最大负荷(P〈0.01)和恢复刚度,Sky骨扩张器组与球囊组在骨折椎体的高度恢复方面的差异无显著性意义(P〉0.05)。Sky骨扩张器组与球囊组骨水泥均分布比较均匀、集中。结论Sky骨扩张器与球囊PKP生物力学测量结果和椎体高度恢复等实验结果无显著性统计学差异且骨水泥的渗漏率极低。Sky骨扩张器的操作相对简单,易于控制,且比球囊扩张系统更经济,Sky骨扩张器后凸成形术为椎体压缩骨折疾病的治疗提供了一种新的选择。Objective To compare the efficacy of Sky bone expander system and balloon kyphoplasty in restoration of vertebral body from three aspects: the improvement of vertebral biomechanical properties,the efficacy of vertebral height restoration and intra- operative extravasation of bone cement. Methods Twenty thoracolumbar vertebral bodies of senile osteoporosis patients were in volved in this study. The vertebral compression fracture model was built through axial loading with the universal mechanical testing machine. The vertebral body height was measured before and after fracture immediately. Using Sky bone expander or balloon ky- phoplasty, vertebral body height and maximum load were recorded before and after bone cement injection. Stiffness was also calculated re- spectively. Results Both Sky bone expander and the balloon increased vertebral maximum load (P〈0.01) and restored the stiffness of the vertebral body. There was no significant difference in the restoration of vertebral body height between Sky bone expander and the balloon groups (P〉0.05). Bone cement was distributed evenly,and focused in both of the two groups. Conclusion There is no significant difference in biomechanics results and vertebral height restoration between Sky bone expander system and balloon ky- phoplasty, and the rate of bone cement leakage is extremely low. Sky expander is more simple than operation, easier control and cheaper than balloon expansion system, which may provide a new choice for the treatment of vertebral compression fractures.
