摘 要:目的 探讨能谱CT单能量成像联合适应性迭代重建技术(ASIR)对于泌尿系发育异常的诊断价值。方法 收集29例本院行能谱CT扫描并经手术及临床病理证实的泌尿系发育异常患者的影像学资料,均进行平扫、肾皮质期、肾实质期和延迟期增强的能谱成像(GSI)扫描,获得70 keV虚拟单能量图像(VMS),并使用50%的ASIR进行图像后处理,得到VMS-ASIR图像。对比皮质期、实质期和延迟期VMS-FBP组和VMS-ASIR组图像噪声和信噪比(SNR),由2名医师对图像质量进行主观评分。结果 在皮质期、实质期和延迟期,2组比较CT值差异无统计学意义(P〉0.05);VMS-ASIR组图像噪声低于VMS-FBP组,SNR高于VMS-FBP组(P〈0.05);VMS-ASIR组图像主观评分高于VMS-FBP组,差异有统计学意义(P〈0.05)。在29例患者中发现肾盂输尿管重复畸形10例,肾盂输尿管连接处狭窄7例,巨输尿管3例,输尿管囊肿3例,腔静脉后输尿管2例,输尿管阴道瘘4例。结论 70 keV VMS-ASIR能够降低图像噪声,提高图像质量,更有利于观察各类泌尿系发育异常的特点,为临床提供有利的诊断依据。Objective To explore the diagnostic value of virtual monochromatic spectral CT imaging, together with adaptive sta-tistical iterative reconstruction (ASIR) technique, for urinary dysplasia. Methods 29 patients confirmed with operation and clinical pathology with urinary dysplasia were analyzed retrospectively. The original images underwent gemstone spectral imaging (GSI) CT imaging for renal cortex phase, renal parenchyma phase and delay phase. VMS-ASIR image was post--processed by 70 keV virtual monochromatic spectral image (VMS] with 50% of ASIR. The image noise and signal to-noise ratio (SNR) for each phase were cal culated and compared between VMS-FBP group and VMS-ASIIR group. Image quality was subjectively assessed by two doctors sepa rarely. Results During the renal cortex phase, renal parenehyma phase and delay phase, the CT attenuation between two groups had no statistical difference (P〉0.05) ; the image noise of VMS-ASIR group was lower than that of VMS FBP group, while SNR was higher than VMS-FBP group (P〈0.05); the image subjective scores of VMS-ASIR group were higher than that of VMS-FBP group (P〈20.05). CT urography showed that 10 cases of ureteropelvie repeat deformity, 7 cases of stricture of pyeloureteric june tion, 3 cases of megaureter, 3 cases of ureter cyst, 2 cases of posteaval ureter and 4 cases of ureteral vagina fistula. Conclusion VMS-ASIR images at 70 keV could reduce the image noise, improve the image quality, and help the observation of all kinds of urinary dysplasia, thus providing favorable clinical diagnosis.
