维持性腹膜透析患者血清脑钠肽水平与心力衰竭关系的研究

摘　　要：目的探讨维持性腹膜透析(MPD)患者血浆脑钠肽(BNP)水平与心力衰竭(HF)之间的关系。方法选择健康志愿者40名和行维持性腹膜透析的尿毒症患者80例,于就诊当天早上依问诊、体查、胸片、心脏彩超等评估心功能情况,同时抽取空腹上肢静脉血检测血清BNP、Scr、BUN的水平。结果 180例MPD患者根据心功能情况分成两组:心力衰竭组45例(A组)和心功能正常组35例(B组)。40例健康志愿者作为对照组(C组)。2三组患者年龄、体重、性别比和身高比较均无显著性差异(P>0.05);A组和B组患者腹膜透析时间比较无显著性差异(P>0.05)。3A组与B组的血清BNP水平、Scr、BUN水平均比C组明显增高(P<0.05);A组与B组的Scr或BUN水平比较无显著性差异(P>0.05);A组血清BNP水平比B组明显升高(P<0.05)。结论维持性腹膜透析患者血清BNP水平与心功能情况密切相关,可作为维持性腹膜透析患者心力衰竭发生的预测指标。Objective To investigate the relationship between Serum BNP and heart failure in patients undergoing maintenance peritoneal dialysis （MPD）. Methods 80 uremia patients who underwent MPD and 40 healthy volunteers were enrolled in the study. In the morning of treatment, we assessed their cardiac function according to interrogation, physical examination, chest X - ray and echocardiography while detecting BNP, Set, BUN levels in the fasting venous serum of upper extremity. Results 80 MPD patients were divided into 2 groups ： a heart failure group （Group A, n = 42） and a normal cardiac function group （Group B, n = 38）. 40 healthy volunteers served as a control group （ Group C ）. There were no significant differences in age, weight and sex ratio among 3 groups （ P 〉 0. 05 ）. The time period of maintenance peritoneal dialysis in Group A and B was similar （ P 〉 0. 05 ）. Compared with Group C, the levels of Serum BNP, Scr and BUN were significantly higher in Group A and B （P 〈 0. 05 ）. There were no significant differences of Ser and BUN between Groups A and B （ P 〉 0. 05 ）. The level of Serum BNP was significantly higher in Group A than that in Group B （ P 〈 0. 05 ）. Conclusion The level of Serum BNP may be closely related to the cardiac function and it may be used as a predictor for heart failure in MPD patients.
