短期使用高频振荡通气对重症人感染H7N9禽流感合并ARDS患者氧合的临床观察

摘　　要：目的探讨短期使用高频振荡通气（highfrequencyoscillationventilation，HFOV）对重症人感染H7N9禽流感合并急性呼吸窘迫综合征（ARDS）患者氧合的改善作用。方法回顾性分析2013—03—01～2014-03-01苏州大学附属第一医院ICU收治的合并ARDS的重症人感染H7N9禽流感6例，其氧合指数（PaO2／FiO2）均〈300mmHg，均接受小潮气量加呼气末正压的机械通气治疗。所有患者在机械通气期间接受持续2hHFOV治疗，在接受HFOV治疗前及治疗后30、120min分别行动脉血气分析，计算各时间点患者氧合指数，行自身对照，比较患者行HFOV前后的氧合水平。结果患者在接受HFOV治疗后30min、120min时的氧合指数[（182．17±36．44）mmHg和（263．17±66．39）mmHg]均显著高于常规机械通气时的水平[（119．00±27．24）mmHg]，差异有统计学意义（P〈0．01）；HFOV治疗后120min时的氧合指数显著高于治疗后30min时的水平（P〈0．05）。结论对于重症人感染H7N9禽流感合并ARDS的患者，短时间（2h）使用HFOV治疗可明显改善氧合。Objective To investigate the value of high frequency oscillation ventilation （HFOV） improving the oxygenation of severe cases of human infections with Avian Influenza A （H7N9 ） who were complicated by acute respiratory distress syndrome （ARDS）. Methods Retrospective analysis severe cases infectived by Avian Influenza A （H7 N9 ） （6 cases） who were complicated by acute respiratory distresssyndrome（ARDS） between march 2013 to March 2014. the oxygenation index of them was small than 300 mm Hg, everyone accepted mechanical ventilation treatment with low tidal volume and positive end - expiratory pressure, and accepted HFOV for 2 hours during mechanical ventilation, arterial blood samples were collected reaspectively at 0 min, 30 min, 120 min after HFOV. calculate the oxygenation index of every time point. Result the oxygenation index at 120 min （263.17 ± 66.39） mm Hg after HFOV was larger than which in 30 min （ 182.17 ± 36.44） mm Hg （ P 〈 0. 05 ） which lager the 0 min （119.00±27.24）mm Hg（P 〈0.05）. Conclusion Short time（2 hours） HFOV can improve the oxygenation of severe cases of human infections with Avian Influenza A （H7N9 ） who were complicated by acute respiratory distress syndrome（ARDS）.
