摘 要:目的研究1,25-二羟维生素D_3[1,25(OH)_2D_3]对Ig A肾病(Ig AN)大鼠肾组织中白细胞介素-4(IL-4)的影响。方法将造模成功的32只Ig AN大鼠模型随机分为未干预组、1,25(OH)_2D_3治疗组、泼尼松组、1,25(OH)_2D_3+泼尼松组,同时设立正常对照组。应用免疫组织化学的方法检测大鼠肾脏组织中IL-4表达的变化。收集Ig AN大鼠的尿蛋白定量(24 h)及尿红细胞计数等生化指标,分别对各组IL-4表达情况与相应的生化指标进行相关性分析。结果 1尿蛋白定量(24 h):正常对照组(A组)为(6.9±0.6)mg/24 h;未干预组(B组)为(50.8±2.9)mg/24 h;泼尼松组(C组)为(12.1±1.3)mg/24 h;1,25(OH)_2D_3治疗组(D组)为(25.4±1.0)mg/24 h;泼尼松+1,25(OH)_2D_3治疗组(E组)为(9.6±0.6)mg/24 h;经治疗后C、D、E组尿蛋白定量(24 h)较B组均有下降(P<0.05),其中E组下降最显著(P<0.05);2尿红细胞计数:A组为(3.3±1.5)个/μl;B组为(377.4±11.1)个/μl;C组为(19.3±2.7)个/μl;D组为(196.6±7.3)个/μl;E组为(9.3±2.2)个/μl;经治疗后C、D、E组尿红细胞计数较B组均有下降(P<0.05),其中E组下降最显著(P<0.05);3IL-4平均光密度值:A组为0.080±0.030,B组为0.230±0.013,C组为0.162±0.021,D组为0.199±0.031,E组为0.092±0.024;经治疗后C、D、E组平均光密度较B组均有下降(P<0.05),其中E组下降最显著(P<0.05);4IL-4的表达与尿蛋白定量(24 h)呈正相关(P<0.05),与尿红细胞计数无明显相关性。结论 1,25(OH)_2D_3下调了肾组织内的IL-4的表达,调节Ig AN大鼠的自身免疫性炎症反应。Objective To investigate the effect of 1,25-dihydroxyvitamin D3 [1,25(OH)2D3] on the expression of interleukin-4 (IL-4) in renal tissues of rats with IgA nephropathy(IgAN). Methods After successful modeling, 32 I- gAN rat models were randomly divided into the nonintervention group, 1,25(OH)2D3 treatment group, prednisone group, 1,25(OH)2D3+prednisonc group, and normal control group. Immunohistochemistry was used to determine the changes of the IL-4 expression in renal tissue of rats. The biochemical indicators of 24-hour urinary protein and red blood cell count in IgAN rats were collected. The IL-4 expression and the corresponding biochemical indicators in each group were performed for correlation analysis in each group, respectively. Results ①The 24-hour quantitative urinary pro-tein in the normal control group (group A), nonintervention group (group B), prednisone group (group C), 1,25(OH)2D3 treatment group (group D) and 1,25(OH)2D3+prednisone group (group E) was (6.9±0.6)mg/24 h, (50.8±2.9) mg/24 h, (12.1±1.3)mg/24 h, (25.4±1.0)mg/24 h, (9.6±0.6)mg/24 h, respectively. After treatment, the urinary protein in group C, D and E were decreased compared with that in group B (P〈0.05), and group E decreased most significantly (P〈0.05). ②The urinary red blood cell count in group A, B, C, D and E was (3.3±1.5)/μ1, (377.4±11.1)/μl, (19.3±2.7)/μl, (196.6±7.3)/μl, and (9.3±2.2)/μl, respectively. After treatment, the urinary red blood cell count in group C, D and E were decreased compared with that in group B (P〈0.05), and group E decreased most significantly (P〈0.05). ③The IL- 4 mean optical density in group A, B, C, D and E was 0.080±0.030, 0.230±0.013, 0.162±0.021, 0.199±0.031, and 0.092±0.024, respectively. After treatment, the mean optical density in group C, D and E were decreased compared with that in group B (P〈0.05), and group E decreased most significantly (P〈0.05
