核苷酸类药物治疗慢性乙型肝炎的耐药性分析

摘　　要：目的:探讨核苷酸类药物治疗慢性乙型肝炎的耐药性。方法:对来笔者所在医院诊治的80例患者入院资料进行分析,将其按照随机数字表法分为两组。所有患者均符合慢性乙型肝炎诊断标准,对照组用药前不进行耐药性检测,试验组用药前进行药物耐药性检测,分析两组患者48周的耐药率、血清学应答等指标。结果:试验组12周耐药率0,24周耐药率为5.41%,48周耐药率为8.57%,低于对照组的5.26%、11.11%、14.71%,差异均有统计学意义(P<0.05);试验组24周HBV、DNA转阴率为55.00%,高于对照组的45.00%(P<0.05);试验组48周HBV、DNA转阴率为70.00%,高于对照组的52.50%(P<0.05);试验组24周HBe Ag血清学转换率为20.00%,48周HBe Ag血清学转换率为32.50%,高于对照组(P<0.05)。结论:慢性乙型肝炎发病率较高,采用核苷酸类药物治疗前应加强耐药性分析,为临床合理用药提供指导。Objective： To investigate the drug resistance of nucleotides in the treatment of chronic hepatitis B.Method： The data of 80 cases admitted in our hospital were analyzed, they were randomly divided into two groups.The patients were consistent with the standard treatment of chronic hepatitis B, the control gronp before treatment for the detection of drug resistance, drug resistance testing experimental groups before treatment, drug resistance analysis of two groups of patients for 48 weeks, the serologic response rate index.Result： The experimental group 12 weeks of drug resistance rate was 0, 24 weeks of resistance rate was 5.41%, 48 weeks of drug resistance rate was 8.57%, were lower than those of the control group（12 weeks of drug resistance rate was 5.26%, 24 weeks of resistant rate was 11.11%, 48 weeks of drug resistance rate was 14.71%）（P〈0.05））.The experimental group 24 weeks HBV, DNA negative conversion rate was 55.00%, higher than 45.00% of the control group（P〈0.05）.The experimental group 48 weeks HBV, DNA negative conversion rate was 70.00%, higher than 52.50% of the control group（P〈0.05）.The experimental group 24 weeks HBeAg serum conversion rate was 20.00%, 48 weeks HBeAg serum conversion was 32.50%, were higher than those of the control group（P〈0.05）.Conclusion： Chronic hepatitis B patients with high incidence rate, should strengthen the drug resistance analysis of nucleotide drugs before treatment to provide guidance for clinical rational drug use.
