摘 要:目的:探讨恩经复(鼠神经生长因子)对糖尿病周围神经病变患者运动和感觉神经传导速度的影响,分析血清中胰岛素样生长因子-1(IGF-1)及胰岛素样生长因子结合蛋白-1(IGFBP-1)的变化。方法:将78例糖尿病周围神经病变患者随机分为两组,对照组39例采用甲钴胺肌肉注射治疗,治疗组39例在对照组的基础上加用鼠神经生长因子肌肉注射治疗。两组共治疗4周。测定治疗前后两组患者正中神经、腓总神经的运动神经传导速度(MNCV)和感觉神经传导速度(SNCV)。测定治疗前后血清的IGF-1及IGFBP-1。结果:对照组经过治疗后的正中神经、腓总神经MNCV和SNCV与治疗前相比,差异统计学意义(P<0.05),治疗组经过治疗后的正中神经、腓总神经MNCV和SNCV与治疗前相比有显著差异(P<0.01),经过治疗后,治疗组的正中神经、腓总神经MNCV和SNCV与对照组相比,差异有统计学意义(P<0.05);对照组经过治疗后IGF-1和IGFBP-1水平有升高,但均无统计学意义(P>0.05),治疗组治疗后的IGF-1和IGFBP-1水平与治疗前相比,差异有统计学意义(P<0.05),与对照组治疗后的水平相比,差异统计学意义(P<0.05)。两组患者均无严重不良反应出现。结论:恩经复联合甲钴胺治疗糖尿病周围神经病变患者安全且效果良好。Objective: To investigate the effect of the nerve growth factor on the motor and sensory nerve conduction velocity in patients with diabetic peripheral neuropathy, and to analyze the changes of -1(IGF-1)and insulin-like growth factor binding protein-1(IGFBP-1)in serum.Methods:78 patients with diabetic peripheral neuropathy were randomly divided into two groups, 39 cases in the control group were treated with intramuscular injection of mecobalamin treatment, 39 cases of the treatment group on the basis of the control group with mouse nerve growth factor muscle injection in the treatment. The two groups were treated for 4 weeks. The motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)were measured in two groups of patients before and after treatment. Serum IGF-1 and IGFBP-1 were measured before and after treatment.Results:The control group were treated with SNCV and MNCV after treatment. The treatment group was significantly different from the control group(P<0.05). After treatment, the median nerve, MNCV and SNCV were significantly different(P<0.05). After treatment, SNCV and MNCV were significantly different from the control group(IGFBP-1 0.01). The control group had statistically significant difference(P<0.05).). No serious adverse reactions occurred in the two groups. Conclusion:The complex effect of mecobalamin in the treatment of diabetic peripheral neuropathy is safe and good.
