摘 要:目的探讨男性2型糖尿病患者血浆致动脉粥样硬化指数(AIP)与胰岛素抵抗(IR)、腹腔内脏脂肪面积(VFA)的关系。方法选择138例男性2型糖尿病患者,所有观察对象进行体格检查、血清学检查、CT测量VFA。以稳态模型评估的胰岛素抵抗指数(HOMA.IR)2.8为切点,分为胰岛素抵抗组(2-DM-IR组)和非胰岛素抵抗组(2-DM.NIR组),比较两组间的临床、生化指标、AIP、VFA。分析AIP与临床、生化指标、VFA的相关性。结果2-DM.IR组舒张压、体质指数(BMI)、腰围、糖化血红蛋白(HbAlc)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、VFA、AIP较2-DM-NIR组均明显升高(p均〈0.05)。相关分析表明,AIP与BMI、腰围(WC)、腰臀比(WHR)、TC、TG、HOMA-IR、VFA正相关(P均〈0.05),与HDL.C(P〈0.01)负相关。多元逐步线性回归分析表明,TG、HDL-C、WC是AIP的独立影响因素(P均〈0.01)。结论在男性2型糖尿病患者中,AIP与BMI、WC、WHR、TC、TG、HOMA-IR、VFA关系密切。腹型肥胖是糖尿病大血管病变的独立危险因素。Objective To investigate the relationship between atherogenic index of plasma ( AlP ) and insulin resistence or visceral fat area ( VFA ) on CT in male patients with type 2 diabetes mellitus.Methods 138 male type 2 diabetic patients were enrolled in the study, the clinical and biochemical data were collected and CT was used to assess visceral fat area. All subjects were divided into 2-DM-IR group and 2-DM-NIR group according to the cutpoint of HOMA-IR at 2.8.The clinical parameters, biochemical data and AIP were compared between two groups. The relationships between AIP and clinical and biochemical data were analyzed. Results Diastolic blood pressure ( DBP ) , body mass index ( BMI ) , waist circumference ( WC ) , glycosylated hemoglobin Alc( HbAlc ) , total cholesterol ( TC ) , serum triglycerride ( TG ) , low density lipoprotein- cholesterol ( LDL-C ) , visceral fat area ( VFA ) , atherogenic index of plasma ( AIP ) were higher in 2-DM-IR group than those in 2-DM-NIR group ( AllP〈0.05 ) . AIP was positively correlated with BMI, WC, WHR, TC, TG, HOMA-IR~ VFA ( all P〈0.05 ) , while negatively correlated with HDL-C ( P〈0.01 ) . Stepwise multiple regression analysis indicated that TG, HDL-C and WC were independent risk factors for AIP ( All P〈0.01 ) . Conclusions The level of AIP is closely correlated with BMI, WC, WHR, TC, TG, HOMA-IR and VFA in male patients with type 2 diabetes mellitus.The increased WC is an independent risk factor for type 2 diabetic macrovascular disease.
