微型T板固定治疗肘关节恐怖三联征临床疗效观察

摘　　要：目的探讨微型T板固定治疗肘关节恐怖三联征的临床疗效。方法 2011年6月至2014年6月我院收治的15例肘关节恐怖三联征患者,均采用手术切开复位内固定治疗,尺骨冠突及桡骨头均采用微型T板固定,并缝合修补内外侧副韧带。术后屈肘90°前臂中立位固定2周,进行功能锻炼。结果 15例患者平均随访14个月,骨折愈合,肘关节无不稳定发生,无疼痛,肘关节屈伸功能平均120°,前臂旋前/旋后平均84°/65°。Morrey肘关节功能评分优8例、良5例、可2例,优良率86.7%。结论微型T板固定桡骨头及尺骨冠突骨折解决了粉碎骨折固定困难的问题,同时修补内外侧副韧带,最大限度恢复肘关节的稳定结构,可早期进行功能锻炼。Objective To investigate the clinical effect of mini T plate fixation for-the treatment of terrible triad of the elbow. Methods Fifteen patients with elbow joint terrorist triple syndrome were treated in our hospital from June 2011 to 2014 June. The patients were all treated with surgical open reduction and internal fixation. The ulna coronoid process and the radius head were fixed with a mini T plate and the medial and lateral collateral ligaments were repaired with suture. After 2 weeks of the 90 （fixation of elbow flexion at forearm neutral position,exercise was performed. Results The average time of follow-up was 14 months. The fracture was healed, and the elbow joints were all stable. There was no pain. The elbow flexion and extension function was 120 （, and the forearm rotation was 84/65. The excellent score of Morrey elbow function score was found in 8 cases, good in 5 cases and qualified in 2 cases. The fine rate was 86. 7%. Conclusion The fixation of the radial head and ulnar fracture with the min T plate can solve the problem of comminuted fractures fixation, and repair the medial collateral ligament, thus, maximally recover the stable structure of the elbow. This can make early functional exercise.
