摘  要:目的探讨新生儿肺透明膜病(hyaline membrane disease,HMD)使用肺表面活性物质(pulmonarysurfactant,PS)的最佳给药方法。方法选择2012年1月至2014年5月我院新生儿科使用PS的HMD新生儿40例,分为对照组15例和观察组25例,对照组采用传统气管导管给药法,观察组于消毒气管导管后使用头皮针头将药物注入气管导管内,立即行复苏气囊正压通气,使药物充分弥散到各侧肺内。比较两组患儿各项指标的变化及治疗效果。结果观察组患儿Sp O2明显高于对照组,无唇周及肢端发绀,无药物返流,给药时间及住院时间短于对照组,症状缓解率高于对照组,肺漏气发生率低于对照组,差异有统计学意义(P<0.01,P<0.05)。结论采用消毒气管导管后用头皮针头注入气管导管内,立即行呼吸气囊正压通气的方式,对患儿SPO2没有明显变化,无药物返流,给药时间短,值得推广。Objective To investigate the best administration method of pulmonary surfactant (PS)in neonates with hyaline membrane disease(HMD). Methods Forty HMD cases from January 2012 to May 2014 in our department were divided into control group( n = 15 )and observation group( n = 25 ). In the control group, PS was given by using traditional endotracheal administration method while the observation group was given by using a new administration method in which drugs was injected into an endotracheal tube with a needle and then the recovery balloon positive pressure ventilation was immediately carried out to make drug sufficient dispersion in each side of lung. The changes of various indexes and curative effects were compared between the two groups. Results The SPO2in the observation group was significantly higher than that in the control group. Patients in the observation group had no lip and acra cyanosis and drug regurgitation. The delivery time and hospital stay time in the observation group were shorter than that in the control group. The observation group also had higher symptom remission rate and lower pulmonary air leakage rate when compared to the control group. All the differences were statistically significant(P 〈 0. 01 ). Conclusion The new administration method makes adequate dispersion of PS into each side of lungs with a little effect on SPO2 and drug regurgitation and reduces drug delivery time. It's worth promoting.
