摘 要:目的了解2岁以下儿童维生素D及骨密度营养状况及其影响因素。方法选取2014年1月至2014年12月就诊于南通市妇幼保健院儿童保健科的2岁以下儿童作为研究对象,采用酶联免疫法测定血清25-羟基维生素D[25-(OH)D]水平,定量超声仪测定骨密度。结果2岁以下儿童血清25-(OH)D水平为53.58±25.06nmol/L,骨密度z值水平为一0.37±1.11。男女童的骨密度z值有明显差异(t=2.322,P〈0.05)。1—2岁组骨密度z值明显高于0~〈1岁组(t=-12.173,P〈0.001),0-〈1岁组25·(OH)D水平明显高于1—2岁组(t=2.238,P〈0.05)。影响25-(OH)D水平的主要因素为户外活动时间、近3月是否补充维生素AD(VitAD)(OR值分别为8.732和0.035,均P〈0.001);影响骨密度z值的主要因素为喂养方式、近3月是否补充钙剂(OR值分别为8.732和0.035,均P〈0.001)。结论2岁以下儿童维生素D水平普遍不足,其中,1~2岁组儿童更为严重;2岁以下儿童骨密度z值普遍正常。儿童保健科医师应加强有关佝偻病预防相关知识的宣教。Objective To investigate the nutritional status for vitamin D and bone mineral density (BMD) and their influencing factors of children under 2 years old. Methods Children under 2 years old who visited child healthcare department from January to December in 2014 were selected as the objects of study. Serum 25-(OH) D level was measured by ELISA, and individual BMD was measured by quantitative ultrasound. Results Serum 25-(OH) D level was 53.58 + 25.06nmol/L and Z value of BMD was -0.37 + 1.11. There was significant difference in Z value of BMD between boys and girls ( t = 2. 322, P 〈 0.05 ). Z value of BMD in 1 - 2 years old group was significantly higher than that in 0 - 1 year old group ( t = - 12. 173, P 〈 0. 001 ), while serum 25-(OH) D level in 0 - 1 year old group was significantly higher than that in 1 - 2 years old group ( t = 2.238, P 〈 0.05 ). Time for outside activities and whether VitAD being added in the last 3 months were the main influencing factors of serum 25-(OH) D level ( OR value was 8. 732 and 0. 035, both P 〈 0. 001 ) , while feeding mood and whether calcium powder being added in the last 3 months were the main influencing factors of Z value of BMD ( OR value was 8. 732 and 0.035, respectively, both P 〈 0. 001 ). Conclusion The serum vitamin D level is generally deficient, and it is worse in 1 - 2 years old group. The Z value of BMD is normal in children under 2 years old. Child heahhcare practitioners should spread knowledge on preventing rickets of vitamin D deficiency.
