新生儿病毒性心肌炎血清ADP、CK-MBmass、IL-6变化及相关性分析

摘　　要：目的探讨新生儿病毒性心肌炎血清脂联素(ADP)与肌酸激酶同工酶质量(CK-MBmass)、白介素-6(IL-6)变化研究及其相关性。方法 85例新生儿病毒性心肌炎患儿,分为急性期组(52例)与恢复期组(33例);另选择42例健康新生儿作为正常对照组。采用双抗体夹心ELISA法检测血清ADP与CK-MBmass、IL-6含量并分析其相关性。多样本对比方差齐性者应用单因素方差分析,组间两两对比应用秩变换后的LSD法。结果急性期组患儿血清ADP含量显著低于恢复期组和正常对照组,均具有统计学差异(t值分别9.139、11.753,均P<0.05);恢复期组血清ADP含量显著低于正常对照组,且具有统计学差异(t=2.648,P<0.05);急性期组患儿血清CK-MBmass含量显著高于恢复期组和正常对照组,均具有统计学差异(t值分别20.890、23.670,均P<0.05);急性期组患儿血清IL-6含量显著高于恢复期组和正常对照组,均具有统计学差异(t值分别11.946、13.675,均P<0.05);恢复期组与正常对照组患儿血清CK-MBmass含量比较无统计学差异(t=0.369,P>0.05);恢复期组与正常对照组患儿血清IL-6含量比较无统计学差异(t=0.388,P>0.05)。急性期组、恢复期组、正常对照组三组新生儿血清ADP均与CK-MBmasss呈负相关性(r值分别为-0.815、-0.859、-0.946,均P<0.05);急性期组、恢复期组、正常对照组三组新生儿血清ADP均与IL-6呈负相关性(r值分别为-0.784、-0.806、-0.914,均P<0.05)。结论 ADP参与新生儿病毒性心肌炎发病过程,可作为诊断疾病、判断病情及治疗效果的参考指标。Objective To explore the changes of serum ADP, CK-MBmass and IL-6 levels in neonates with viral myocarditis and their correlation. Methods Totally 85 neonates with viral myocarditis were selected and divided into acute phase group （52 cases） and recovery phase group （33 cases）. Another 42 healthy neonates were selected as controls. Double-antibody sandwich ELISA was used to detect serum ADP, CK-MBmass and IL45 levels and their correlation was analyzed. One-way ANOVA and LSD method were used for analysis. Results Serum ADP level in the acute phase group was significantly lower than the recovery phase group and the control group （ t value was 9. 139 and 11. 753, both P 〈 0, 05 ） , and that in the recovery phase group was significantly lower than the control group （ t = 2. 648,P 〈 0.05 ）. Serum CK-MBmass and IL-6 levels in the acute phase group were higher than those in the recovery group and the control group with significant differences （t CK-MBmass value was 20. 890 and 23. 670, respectively,t IL45 value was 11. 946 and 13. 675, respectively, all P 〈 0.05 ）. Serum CK-MBmass and IL-6 levels were not significantly different between the recovery phase group and the control group （ t value was 0. 369 and 0. 388, respectively, both P 〉 0.05）. In three groups serum ADP level was negatively correlated with CK-MBmass and IL- 6 （r CK-MB value was - 0. 815, - 0. 859 and - 0. 946, respectively;r IL-6 value was - 0. 784, - 0. 806 and - 0. 914, respectively, all P 〈 0.05 ）. Conclusion ADP participates in the pathogenesis of neonatal viral myocarditis, which can be considered as a reference index for diagnosing disease, evaluating the condition and therapeutic effect.
