金昌队列人群高血压的疾病负担趋势性研究

摘　　要：目的了解高血压对金昌队列人群造成的疾病负担及其变化趋势,为金昌队列人群制定有针对性的高血压防治措施、减轻该人群高血压的疾病负担提供基础数据。方法采取历史队列研究方法收集金昌队列人群2001-2013年高血压全死因资料及2001-2010年高血压住院患者资料,应用死亡率、直接经济负担、潜在减寿年数(PYLL)、平均潜在减寿年数(APYLL)、潜在工作损失年数(WYPLL)以及人均潜在工作损失年数(AWYPLL)等指标分析高血压造成的疾病负担现状。结果 2001-2013年金昌队列人群高血压粗死亡率为16.73/10万,男性高血压死亡率为24.07/10万,平均增长速度为42.40%。因高血压而造成的PYLL为786.78年、APYLL为8.46年/人、WPYLL为329年和AWPYLL为3.54年/人,13年间无增长趋势。2001-2010年金昌队列人群高血压住院患者人均直接经济负担为2 385.04元,日均直接经济负担为176.38元,10年间无增长趋势。结论高血压对金昌队列人群造成沉重的疾病负担,是重要的健康问题。Objective To evaluate the disease burden of hypertension and its changing trend in the Jinchang cohort, to develop targeted prevention and control measures for hypertension, to provide scientific evidence for reducing the disease burden. Methods The data on causes of death from hypertension from 2001 to 2013 and medical records of hospitalized patients with hypertension from 2001 to 2011 were collected in the historical retrospective cohort study. Mortality, direct economic burden, potential years of life lost （PYLL）, average potential years of life lost （APYLL）, work potential years of life lost （WPYLL） and average work potential years of life lost （AWPYLL） were used to analyze current status of the disease burden caused by hypertension. Results From 2001 to 2013, the crude mortality for hypertension was 16.73 per 100 000 and 24.07 per 100 000 for males in the Jinchang cohort with the average increase speed of 42.40%. The PYLL, APYLL, WPYLL, AWPYLL of hypertension were 786.78 years, 8.46 years per person, 329 years and 3.54 year per person respectively with no obvious increase during 13 years. From 2001 to 2010,the direct economic burden of hospitalized patients with hypertension is 2 385.04 yuan per person and 176.38 yuan per day, with no obvious increase during 10 years. Conclusion Hypertension as major health problem leads to heavy burden of disease in the Jinchang Cohort.
