丹参多酚酸盐联合西药治疗冠心病不稳定型心绞痛的临床疗效及对血清NO、ET-1的影响

摘　　要：目的观察丹参多酚酸盐联合西药治疗冠心病不稳定型心绞痛的临床疗效及对血清一氧化氮(NO)、内皮素-1(ET-1)的影响。方法将88例老年冠心病不稳定型心绞痛患者随机分为观察组及对照组各44例,对照组给予常规西药治疗,观察组在对照组治疗基础上给予丹参多酚酸盐治疗,连续治疗2周。观察2组治疗期间心电图中ST段变化及临床症状改善情况,并比较2组治疗前后的血清NO、ET-1水平。结果观察组在治疗后1,2周的ST段下移程度及缺血导联数目均较对照组降低(P均<0.05);观察组在治疗后1,2周的心绞痛发作次数、硝酸甘油使用量均少于对照组(P均<0.05);观察组治疗后心电图疗效和心绞痛疗效均优于对照组(P均<0.05);2组治疗后血清NO水平均有所升高,ET-1水平降低,但观察组血清NO水平高于对照组,ET-1水平低于对照组,差异均有统计学意义(P均<0.05);2组治疗期间均未出现不良反应。结论丹参多酚酸盐联合西药治疗冠心病不稳定型心绞痛的临床效果显著,且安全可行,能够显著提高ET-1水平,抑制NO水平,对改善内皮功能具有一定作用。Objective It is to observe the clinical effect of salvianolate combined with western medicine on unstable angina pectoris and effect on serum NO - 1 and ET - 1. Methods 88 elderly patients with unstable angina pectoris were randomly divided into observation group and control group, each group had 44 cases. The control group was given normal western medicine therapy, on this basis, the observation group was added with salvianolate. Both the groups were treated for 2 weeks. The changes of ST segment in ECG and improvements of clinical symptoms were observed in both groups during treatment, and the lev- els of serum NO and ET - 1 before and after treatment were compared between the two groups. Results The decreasing degree of ST segment, numbers of ischemic lead, attack times of angina pectoris, using dosage of Nitroglycerin after one and two week ＂s treatment in observation group were lower or less than that in control group （P all 〈 0.05）. The curative effect of ECG and angina pectoris in observation group were better than that in control group （ P all 〈 0.05 ）. After treatment, the levels of serum NO increased, that of ET - 1 decreased in both groups, but the decrease or increase in observation group were more obviously than that in control group, the differences were significant （P all 〈 0.05）. No side effect was found during treatment in both groups. Conclusion Salvianolate combined with western medicine has a good clinical effect on unstable angina pectoris with good safety, which can significantly increase the level of ET - 1 and inhibit the level of NO, and has some improving function on endothelial dysfunction.
