老年患者腹部手术后肺部感染防治的护理干预

摘　　要：目的探讨加强全面护理干预老年患者腹部手术后肺部感染的发生率。方法将该院收治的240例老年腹部手术患者随机分为观察组与对照组,对照组患者采用常规护理措施,观察组患者使用加强全方位护理干预,比较2组患者术后24h排痰量、肺部感染发生率、住院时间、护理满意度。结果观察组患者术后4d内24h排痰量[(22.63±5.66)、(33.41±8.36)、(34.25±8.57)、(34.72±8.69)mL]均显著高于对照组[(15.23±3.81)、(17.92±4.48)、(18.06±4.52)、(18.66±4.67)mL],观察组患者术后肺部感染发生率(2.50%)明显低于对照组(7.50%),住院时间[(11.74±3.05)d]短于对照组[(13.86±3.47)d],护理满意度(93.33%)高于对照组(78.33%),差异均有统计学意义(P<0.05)。结论老年患者腹部手术实施全面护理干预,可促进术后早期排痰,有效降低肺部感染发生率,缩短住院时间,提高护理工作满意度,值得临床推广应用。Objective To investigate the effect of intensive postoperative airway nursing on pulmonary infection prevention in elderly patients undergone abdominal operation.Methods 120 patients were divided as the observation group that treated from January 2013 toJanuary 2014,and 120 patients were recruited into the control group from December 2011 to December 2012.The control group were treated with normal postoperative nursing pattern,while the observation group were treated with intensive airway secretion and atomization inhalation nursing pattern on the basis of the control group.The 24 hsputum volume after operation,the incidence rate of pulmonary infection,the hospital stays and the nursing satisfaction of the patients were evaluated.Results 120 patients were divided as the observation group that treated from January 2013 toJanuary 2014,and 120 patients were recruited into the control group from December 2011 to December 2012.The control group were treated with normal postoperative nursing pattern,while the observation group were treated with intensive airway secretion and atomization inhalation nursing pattern on the basis of the control group.The 24 hsputum volume after operation,the incidence rate of pulmonary infection,the hospital stays and the nursing satisfaction of the patients were evaluated.Conclusion Intensive postoperative airway nursing on elderly patients undergone abdominal operation had satisfying effect of early postoperative sputum discharge,pulmonary infection prevention,hospital days reduction,and the nursing satisfaction improvement.It was worth further promotion in the clinical practice.
