罗铨教授生脉温胆汤治疗肺心病急发期临床观察  

Clinical Observation of Shengmai Wendan Decoction Prescribed Prof. Luo Quan on the Treatment of Cor Pulmonale during Acute Stage

摘　　要：目的观察生脉温胆汤治疗慢性肺源性心脏病(以下简称肺心病)急性期的临床疗效。方法将104例肺心病急性期患者随机分为2组,对照组52例给予常规西医治疗,治疗组52例在对照组治疗基础上予中药汤剂治疗,2组均治疗14天后观察治疗前后中医症状积分、西医体征积分变化、综合疗效评定、血气分析。结果治疗组总有效率96.15%,对照组总有效率82.69%,2组总有效率比较差异有显著性意义(P<0.05),治疗组疗效优于对照组。2组治疗后中医症状积分、西医体征积分均较本组治疗前降低(P<0.05),治疗组治疗后中医症状积分、西医体征积分均低于对照组(P<0.05)。2组治疗后氧分压(Pa O2)及二氧化碳分压(Pa CO2)均改善(P<0.05),治疗组改善优于对照组(P<0.05)。结论肺心病急性期采用生脉温胆汤治疗能明显提高总体疗效,改善症状、体征、血气分析结果,提高患者生活质量。Objective ： To observe the clinical effect ofShengmai Wendan Decoction on the treatment of chronic pulmonale （hereinafter referred to as pulmonary heart disease） during acute stage. Methods： 104 patients were ran- domly divided into a treatment group and a control group, 52 cases per group. The control group was only treated with conventional western medicine while the treatment group was added withShengmai Wendan Decoction besides the con- ventional western medicine for 14 days. The TCM symptom scores, physical sign score changes of western medicine, comprehensive treatment effect evaluation and blood gas analysis of the two groups were observed before and after the treatment. Results ： The total effectiveness rate of the treatment group was 96. 15% while that of the control group was 82. 69%. The comparison of the total effectiveness rates between the two groups was significant difference （ P 〈 0. 05 ） The treatment group was better than the control group. The TCM symptom scores and physical sign scores of the two groups dropped when compared with the pre - treatment （ P 〈 0. 05）, but the TCM symptom scores and physical scores of the treatment group was lower than that of the control group （ P 〈 0. 05 ）. The oxygen partial pressure （ Pa si O and carbon dioxide partial pressure （ PaCO2 ） of the two groups improved （P 〈 0. 05 ）. The treatment group improved better than the control group （P 〈 0. 05 ）. Conclusion ： Shengmai Wendan Decoction can significantly improve the over- all effect for the treatment of pulmonary heart disease during acute stage and improve symptoms, signs and blood gas, thus improving the quality of life of patients.
