摘  要:目的:探讨糖尿病周围神经病变(DPN)的危险因素。方法:选取本院73例老年糖尿病住院患者作为研究对象,运用肌电图检测其肢体运动神经传导速度(MCV)以及感觉神经传导速度(SCV),统计神经传导速度的异常率,并对疾病相关危险因素进行分析。结果:腓总神经MCV改变与糖化血红蛋白(Hb A1c)、病程、收缩压明显相关(P<0.05),且危险程度为糖化血红蛋白>病程>收缩压;胫神经MCV与收缩压、糖化血红蛋白、空腹血糖相关(P<0.05),且危险程度为收缩压>糖化血红蛋白>空腹血糖;腓浅神经SCV的改变与收缩压、年龄、病程相关(P<0.05),且危险程度为收缩压>年龄>病程。结论:病程、年龄、收缩压、糖化血红蛋白、空腹血糖是糖尿病患者发生周围神经病变的危险因素,神经传导速度检测是早期发现糖尿病周围神经病变的有效手段。Objective:To investigate the risk factors of diabetic peripheral neuropathy(DPN).Method:73 hospitalized patients with diabetes in our hospital were selected as the research objects.Electromyography was applied to examine the motor nerve conduction velocity(MCV) and the sensory nerve conduction velocity(SCV) for these patients.The abnormal rate of the nerve conductive velocity was counted and the possible risk factors of the disease were analyzed.Result:Changes in the MCV of common peroneal nerve were significantly correlated with glycosylated hemoglobin,duration and systolic blood pressure(P〈0.05).The dangerous degree was glycosylated hemoglobindurationsystolic blood pressure.Changes in the MCV of tibial nerve were related to systolic blood pressure,glycosylated hemoglobin and fasting blood sugar(P〈0.05).The dangerous degree was systolic blood pressureglycosylated hemoglobinfasting blood sugar.Changes in the SCV of superficial peroneal nerve were associated with systolic blood pressure,age and duration(P〈0.05).The dangerous degree was systolic blood pressureageduration.Conclusion:Duration,age,systolic blood pressure,glycosylated hemoglobin and fasting blood glucose are the risk factors of DPN.Nerve conductive velocity test is an effective means of early detection of DPN.
