摘  要:目的了解肿瘤患者血培养阳性致病菌的病原茵分布及药敏情况,为临床合理使用抗菌药物提供参考。方法采用双相血培养瓶进行人工培养,所有菌株按照常规操作流程进行鉴定,药敏试验采用K-B纸片扩散法,用WHONET5.4软件对药敏结果进行分析。结果1214例血培养标本中共分离出102株病原菌,检出率为8.4%;其中摹兰阳性球菌34株,占33.3%,革兰阴性杆菌64株,占62.7%,真菌3株,占3.0%。耐药性分析显示,大肠埃希菌和肺炎克雷伯菌对头孢哌酮-舒巴坦、哌拉西林/他唑巴坦和阿米卡星的耐药率均不高于20%,对亚胺培南保持高度敏感。铜绿假单胞菌对亚胺培南和阿米卡星较敏感。未检出糖肽类或利奈唑胺耐药的革兰阳性球菌。结论血培养病原茼菌谱较广,以革兰阴性杆菌为主要病原茼,临床分离菌株对多数常用抗菌药物耐药性普遍存在。重视不同病区的痛原菌的监测和合理用药具有重要意义。Objective To investigate the distribution and antimicrobial susceptibility of pathogens of tumor patients with blood-positive cultured,and to provide reference for clinical rational use of antimicrobial drugs. Methods Using biphasic blood culture bottles to culture. All strains were identified in accordance with normal operating procedures. Using WHONET5.6 software to analyze the susceptibility results. Results 102 pathogen were isolated from 1214 cases of blood culturing samples,gram-positive cocci accounting for 33.3 % ,gram-negative bacilli accounting for 62.7 %, fungi accounting for 3.0 %. Resistance analysis showed that the resistance rate of E. coli and Klebsiella pneumoniae to cefoperazone-sulbactam,piperacillin / tazobactam and amikacin is not higher than 20%, and remain highly sensitive to imipenem. Pseudomonas aeruginosa is most sensitive to imipenem and amikacin. Undetected glycopeptide or linezolid-resistant Gram-positive cocci. Conclusion Gram-negative bacteria are common among the micro-organisms isolated from blood culture and their resistance are quite different. Attaches great importance to the different ward pathogen monitoring and rational drug use is of great significance.
