“双C”方案治疗难治性糖尿病的临床观察

摘　　要：目的比较"双C"方案(动态血糖监测联合皮下胰岛素泵持续输注)与"MSⅡ"方案(多次皮下注射胰岛素)治疗难治性糖尿病的疗效及其对胰岛素用量、胰岛素抵抗指数的影响。方法选择2013年1月—2014年4月收治的难治性糖尿病80例,按治疗方法分为观察组和对照组,每组40例。观察组给予"双C"方案治疗,对照组给予"MSⅡ"方案治疗。比较两组临床疗效、低血糖发生次数、胰岛素用量及胰岛素抵抗指数。结果两组治疗后血糖水平均低于治疗前,且观察组低于对照组(P<0.05)。观察组血糖达标时间短于对照组,胰岛素抵抗指数低于对照组,胰岛素用量及低血糖发生次数少于对照组(P<0.05)。结论 "双C"方案治疗难治性糖尿病血糖水平控制更佳,能明显减少胰岛素用量,降低低血糖事件发生率。Objective To compare the therapeutic effect of ＂ double C＂ （dynamic blood glucose monitoring combined with continuous subcutaneous insulin infusion） and ＂MS II ＂ （multiple subcutaneous insulin injection） methods in treatment of intractable diabetes and their effects on insulin dosage and resistance index. Methods A total of 80 patients with refractory diabetes admitted during January 2013 and April 2014 were divided into observation group （ n = 40） and control group （n = 40） according to therapeutic methods. The observation group was treated with ＂ double C＂ method, while the control group was treated with MS II method. The clinical efficacy, the incidence frequency of hypo- glycemia, insulin dosage and resistance index were compared between the two groups. Results The levels of blood glu- cose in the two groups were lower than those before the treatment, and the level in the observation group was lower than that in the control group （ P 〈 0.05 ）. In the observation group, the time up to the standard of blood glucose was shorter, the insulin resistance index was lower, and the values of insulin dosage and incidence frequency of hypoglycemia were lower than those in the control group （P 〈 0. 05 ）. Conclusion The ＂ double C＂ method can significantly reduce the in- sulin dosage and the incidence frequency of hypoglycemia in treatment of patients with refractory diabetes.
