摘 要:目的观察门冬胰岛素治疗妊娠期糖尿病(GDM)外周血清氧化相关物质水平的变化情况及其疗效,评估其安全性。方法选择2012年1月—2015年1月收治的GDM 80例,随机分为观察组和对照组,每组40例。对照组给予生物合成人胰岛素治疗,观察组在此基础上给予门冬胰岛素。观察两组治疗后血糖相关指标的变化情况,比较治疗前与治疗后第5天外周血清氧化相关物质浓度,评估不良反应及母婴结局。结果两组治疗1、3、5 d后空腹血糖(FPG)、餐后2 h血糖(2 h PG)均较治疗前有所下降(P<0.05)。两组治疗第3、5天三餐后2 h PG均低于治疗第1天(P<0.05)。观察组治疗第3天午餐后2 h PG低于对照组(P<0.05)。两组治疗5 d后超氧化物歧化酶、谷胱甘肽过氧化物酶活性均较治疗前升高,丙二醛、活性氧类物质均较治疗前降低(P<0.05);观察组治疗5 d后外周血MDA浓度高于对照组(P<0.05)。两组临床疗效、不良反应及母婴不良结局发生率比较差异无统计学意义(P>0.05)。观察组满意度优于对照组(P<0.05)。结论门冬胰岛素联合生物合成人胰岛素能很好地纠正GDM患者外周血清氧化与抗氧化物质的失衡,安全有效。Objective To observe the effect and content changes of oxidative related substances of Insulin Aspart in treatment of women with gestational diabetes mellitus (GDM) and to evaluate its safety. Methods A total of 80 GDM patients admitted during January 2012 and January 2015 were randomly divided into observation group (n = 40) and control group ( n = 40). The control group was given biosynthetic human insulin, while the observation group was given Insulin Aspart based on biosynthetic human insulin. In the two groups, the index changes of relative blood glucose before and after the treatment were observed, and content changes of oxidative related substances before and on the 5^th d after the treatment were compared, and the incidence rate of adverse effect and outcomes of pregnant women and their babies were also evaluated. Results The levels of fasting plasma glucose (FPG) and 2 h postprandial glucose (2 h PG) were significantly decreased on the 1^st, 3^rd and 5^th d after the treatment compared with those before the treatment (P 〈 0.05 ). The 2 h PG levels after three meals on the 3^rd and 5^th d after the treatment were lower than those on the 1^st d after the treatment (P 〈 0. 05). The 2 h PG level after lunch on the 3rd d after the treatment in the observation group was significantly lower than that in the control group (P 〈 0. 05). On the 5^th d after the treatment in the two groups, the activities of superoxide dismutase (SOD) and glutathione peroxidase (GSH-PX) were significantly increased, and the levels of malondialdehyde (MDA) and reactive oxygen species (ROS) were significantly decreased compared with those before the treatment (P 〈 0. 05). The MDA level in peripheral blood on the 5^th d after the treatment in the observation group was significantly increased compared with that in the control group (P 〈 0. 05).
