摘 要:目的:讨论服毒一血液灌流时长与急性百草枯中毒预后的关系。方法:对在我院行血液灌流医治的50例急性百草枯中毒患者的临床资料进行回顾性分析,据血液灌流时长分三组。A组:服毒一血液灌流时长小于4h;B组:服毒一4h≤服毒一血液灌流时间<8h;C组:8h≤服毒一血液灌流时长<12h。对比A,B,C三组脏器损伤状况、死亡率以及死亡患者存活时间等,分析服毒一血液灌流时间对患者预后的影响。结果:28d内死亡36例。A、B、C三组死亡例数为8例、15例、13例,A、B、C三组死亡率分别为50.00%、70.00%、92.85%,三组28d的病死率差异存在统计学意义(P<0.05)。对比三组间ALTmax、SCrmax、AMYmax和Pa O2min水平。各组患者在急性脏器损伤的差异在统计学上存在意义(P<0.05)。结论:百草枯农药中毒不超过4h开始HP可以显著缓解靶器官受损,减少患者病死率。Objective: To study the relationship of poison-hemoperfusion time and acute paraquat poisoning prognosis. Method: Retrospective analysis was made on the clinical data of 50 cases of acute paraquat poisoning treated in our hospital with HP. According to length of Hp time,they were divided into 3 groups.Group A: the time of P-HP 4h; group B,4h ≤the time of P-HP 8 h; group C,8h≤the time of P-HP 12h. The organ injury death rate and the survival time of the patients died were companed,the effect of P-HP time on the prognosis of the patients was analyzed. Result: 34 cases died in 28 days,and the mortality rate was 68.0%. The death cases of group A,B,C were 11 cases,15 cases and 8 cases.The mortality in 28 days of the three groups was statistically significant( P〈0.05). The levels of Pa O2 min,AMYmax,SCrmax and ALTmax were compared and acute organ injury had statistically significant differences in the three groups( P0.05). Conclusion: Paraquat poisoning dose not exceed 4h,having HP can significantly ease the target organs damage,and reduce mortality of patients.
