石家庄及周边地区结核性脑膜炎的分析

摘　　要：目的探讨结核性脑膜炎（TBM）的临床流行病学和临床特点。方法对河北医科大学第二医院1995至2013年间收治的1029例TBM的住院病例进行分析。结果119年间本院共收治TBM的患者1029例，男女比例1．5：1，发病年龄38．6±15．6岁。TBM患者2004至2008年较1999至2003年增加了11．0％，2009至2013年较2004至2008年增加了52．7％，2013年住院患者为最多，为108例。春季（27．2％）和夏季（27．9％）较秋季（22．7％）和冬季（22．2％）住院患者多，患者主要分布在河北省南部地区，以石家庄（49．76％）、保定（49．76％）、邢台（12．73％）为主，多数患者居住于农村（65．2％），以农民（73．47％）为主，其次为学生（9．7％）。TBM主要集中在20～30岁年龄组（24．5％），其次为40-50岁年龄组（16．23％）。结论石家庄及周边地区TBM确诊患者有明显增加趋势，做好预防工作及早诊断、早治疗。Objective To investigate the epidemiological and clinical characteristics of tuberculous meningitis（TBM）. Methods Inpatients in the second hospital of Hebei Medical University from 1995 to 2013 were subjected into retrospective study.Results During 19 years, the hospitalization number of TBM was 1029. The ratio of male/female was 1.5：1. The distribution of onset age was 38.6＋ 15.6. The hospitalization number of TBM was 11.0% higher in 2004-2008 than in 1999-2003. The hospitalization number of TBM was 52.7% higher in 2009-2013 than in 2004-2008. The number of patients was 108 in 2013, reaching a new level. The number of patients was larger in spring （27.2%） and summer（27.9%） than in autumn（22.7%）and winter（22.2%）. Most of the patients resided in south of Hebei province, especially in Shijiazhuang（49.76% ）, Baoding（49.76%） and Xingtai（ 12.73% ）. Most of them were farmers（ 73.47% ）, followed by students（9.7%）. Nearly two-third （ 65.2% ） of the patients lived in rural areas（65.2%）. TBM was especially common in the 20-30 age group （24.5%）, followed by 40-50 age group （16. 23%）. Conclusion In the past 19 years, the incidence of TBM have increased in Shijiazhuang and its surrounding areas. Our vigilance shall be improved and early diagnosis and treatment are very important.
