摘 要:目的观察瑞舒伐他汀在改善血脂正常原发性高血压患者血管内皮功能中的作用。方法血脂正常高血压1~2级患者324例,随机分为观察1组、观察2组和对照组各108例,观察1组给予瑞舒伐他汀5mg/次,观察2组给予瑞舒伐他汀10rag/次,对照组给予安慰剂1片/次,3组均1次/d,口服,连续12周。分别于治疗前、后测定3组血压、总胆固醇(total cholesterol,TC)、三酰甘油(triacylglycerol,TG)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL—C)、低密度脂蛋白胆固醇(low density lipoprotein-cholesterol, LDL-C)、一氧化氮(nitric oxide,NO)、内皮素(endothelin,ET)、颈动脉内-中膜厚度(intima-media thickness of carotid artery, IMT)、肱动脉内皮依赖性舒张功能(brachial artery endothelial dependent vasodilation, EDV),并进行比较。结果对照组治疗前、后各指标水平比较差异无统计学意义(P〉0.05);治疗后观察1组、观察2组收缩压、舒张压均较治疗前下降,但差异无统计学意义(P〉0.05);观察1组、观察2组治疗后TC[(3.75±0.42)、(3.12±0.35)mmol/L]、LDL-C[(2.70±0.32)、(2.21±0.38)mmol/L]、ET[(132.45±17.98)、(115.21±17.53)mg/L]、IMT[(1,oo±0.02)、(0.75±0.05)mm]水平较治疗前[TC分别为(4.53±0.48)和(4.58±0.47)mmol/L、LDL—C分别为(3.40±0.39)和(3.45±0.39)mmol/L、ET分别为(142.68±19.12)、(141.89±18.78)mg/L、IMT分别为(1.34±0.03)和(1.33±0.04)mm]明显下降,NO[(42.06士10.09)、(53.78i9.12)mg/L]、EDV[(14.25±2.32)%、(18.36±3.18)%]水平较治疗前[NO分别为(36.02±9.21)和(35.42±9.45)mg/L、EDV分别为(8.04±2.39)%和(8.02±2.36)%]增高,差异均有统计学意义(P〈0.05),且观察2组各指�Objective To observe the influence of rosuvastatin on vascular endothelial function in the essential hypertensive patients with normal plasma lipid. Methods A total of 324 patients with hypertension of grade 1 to 2 and normal plasma lipid were randomly divided into observation group 1, observation group 2 and control, group, with 108 patients in each group. Observation group 1 received rosuvastatin 5 mg once a day, observation group 2 received rosuvastatin 10 mg once a day and control group received the placebo one tablet a day, totally for 12 weeks. The blood pressure, total cholesterol (TC), triacylglycerol (TG), high density lipoprotein-cholesterol ( HDL-C), low density lipoprotein-cholesterol (LDL-C), nitric oxide (NO), endothelin (ET), intima-media thickness (IMT) and brachial artery endothelial dependent vasodilation (EDV) were detected before and after treatment and compared among three groups. Results There were no significant differences in the indexes before and after treatment in control group (P〈0.05). The systolic blood pressure and diastolic blood pressure decreased after treatment in observation group 1 and 2, which were not significantly different from those before treatment (P〉0. 05). The levels of TC ((3.75 ±0.42), (3. 12± 0.35) mmol/L), LDL-C ((2.70±0.32), (2.21±0.38) mmol/L), ET ((132.45±17.98), (115.21±17.53)mg/L) and IMT ((1.00 ± 0. 02), (0. 75 ± 0. 05) mm) after treatment were significantly lower than those before treatment (TC:(4.53±0.48), (4. 58±0. 47) mmol/L; LDL-C: (3. 40±0. 39), (3. 45±0. 39) mmol/L, ET: (142. 68±19. 12), (141.89±18.78) rag/L) ; IMT: (1. 34±0.03), (1. 334-0.04) mm) , and the levels of NO ((42. 06±10.09), (53.78±9.12) mg/L) and EDV ((14. 25 ± 2. 32)%, (18. 36 ± 3. 18)%) after treatment were significantly higher than those before treatment (NO: (36. 02±9. 21), (35. 42±9. 45) mg/L
