摘　　要：目的探讨宫颈管长度测量、阴道分泌物细菌学检查及胎儿纤维连接蛋白检测诊断早产的价值与相关性。方法选取健康孕妇50例作为对照组,另选取高危早产孕妇50例作为观察组。对比2组宫颈管长度、阴道分泌物细菌学检查及胎儿纤维连接蛋白水平的差异性。分析不同宫颈管长度的高危早产孕妇的妊娠结局、宫颈管长度与早产的相关性及胎儿纤维连接蛋白预测早产的敏感度、特异性、阳性预测值与阴性预测值。结果观察组宫颈管长度明显短于对照组(P<0.05),宫颈评分显著高于对照组(P<0.05),且观察组中宫颈管长度越短,早产概率越高。观察组阴道细菌感染率为32%,对照组无细菌感染;观察组纤维连接蛋白水平显著高于对照组,纤维连接蛋白预测早产的敏感度为92%,特异性为100%,阳性预测值为100%,阴性预测值为92%。结论对高危早产孕妇进行宫颈管长度测量、阴道分泌物细菌学检查及胎儿纤维连接蛋白检测具有重要的临床价值,有助于医生准确诊断早产,值得推广与应用。Objective It is to explore the value and correlation of cervical length measurement,fetal protein fiber connection testing and vaginal bacteriology examination for the diagnosis of premature birth. Methods 50 healthy pregnant women were selected as control group,50 high-risk preterm pregnant women selected observation group. The differences in cervical length,vaginal bacteriology inspection and fetal protein fiber content were compared between the two groups,The pregnancy outcomes and the correlations of cervical length with premature in high-risk pregnant women were analyzed. The sensitivity of the fiber connection protein prediction on the fetus preterm birth,specificity,positive predictive value and negative predictive value were analyzed. Results The cervical length of pregnant women in observation group was shorter,while the cervical scores were higher than those in control group,with the decreasing of the length,the probability of premature increased in observation group. The bacterial infection rate was 32% in observation group,while no infection was found in control group. The level of fetus fiber connect protein was higher than that in control group. At the same time,the sensitivity of the fiber connection protein for predicting premature birth was 92%,specificity was 100%,positive predictive value was 100%,negative predictive value was 92%. Conclusion Cervical length measurement,premature fetus fiber connection protein detection and vaginal bacteriology detection has important clinical value for the high-risk pregnant women,which can help doctors to accurately diagnose premature birth,and is worthy of popularization and application.
